Il est essentiel que la recherche clinique se perpétue en France et ce pour des raisons à la fois scientifiques et économiques. La France dispose d'avantages qui lui permettent de maintenir une position compétitive à l'échelle internationale. Toutefois, l'enquête du Leem de 2014 souligne des faiblesses comme l'augmentation des délais de contractualisation ainsi qu'une diminution du nombre moyen de patients par étude et du nombre moyen de patients par centre. Le recrutement des patients est une difficulté majeure des essais cliniques et l'émergence de pays d'Asie ou d'Europe de l'Est avec un fort taux d'inclusion ne fait qu'accentuer la problématique en France. Ainsi, nous avons étudié les taux d'inclusion des études à promotion AP-HP réalisées en France et des études industrielles effectuées à l'AP-HP puis affiné notre analyse selon différents critères. Nous avons également sollicité les investigateurs sur la nature des difficultés d'inclusion dans les essais cliniques industriels du Groupe Hospitalier (GH) de l'Est Parisien. Il ressort que les études incluent en moyenne environ deux tiers des patients initialement prévus. Des différences s'observent selon la typologie de l'étude avec des taux d'inclusion plus élevés pour les études non interventionnelles et en soins courants et plus faibles pour les études portant sur des dispositifs médicaux. Certaines aires thérapeutiques, telles que l'oncologie, le métabolisme-nutrition et la virologie, ont des taux d'inclusion au-dessus de la moyenne tandis que d'autres, telles que la neurologie ou la néphrologie, semblent avait plus de difficultés. Les réponses des investigateurs sollicités ainsi que les observations décrites dans la littérature permettent d'identifier plusieurs leviers pour optimiser le recrutement des patients tels qu'un allègement du protocole, une formation accrue des investigateurs et du personnel de recherche ainsi qu'une attribution de moyens suffisant aux centres par le promoteur. Des efforts ont également été entrepris par les pouvoirs publics avec notamment, pour les études académiques, la mise en place du financement par tranche et dont l'analyse tend à montrer un impact positif sur les taux recrutement. Ainsi, il serait intéressant d'évaluer le retentissement d'autres mesures telles que la mise en place du contrat unique ainsi que la création des structures d'aide à la recherche.
