Introduction (contexte et objectifs): Les médicaments représentent une part importante des dépenses de santé dans le monde. Les génériques et les politiques de santé associées, la substitution et la prescription en dénomination commune internationale sont des moyens pour diminuer ces coûts. Cette étude vise à faire un état des lieux de la prescription des génériques dans le monde. Méthode: Une revue de la littérature systématique, réalisée selon la méthode PRISMA en utilisant les bases de données PUBMED, GOOGLE SCHOLAR, BDSP, CAIRN et la littérature grise sur la période de 2003 à 2018 a été effectué, portant sur la prescription des génériques, les politiques de santé, l'industrie du médicament, la substitution et la prescription en DCI. Résultats: Sur les 869 études identifiées, 91 étaient éligibles pour l'inclusion. Elles ont révélé des taux de prescription des génériques plus élevés dans les pays anglo-saxons (Etats Unis, Royaume Uni, Australie...) associés à des taux de substitution et de prescription en DCI plus importants associées à des politiques de santé précoces et favorisantes de l'utilisation des génériques étaient instaurées. D'autres principes favorisent l'utilisation des génériques: Définition légale du générique, faciliter les démarches administratives d'entrée sur le marché, informer les médecins, pharmaciens et patients. Certaines pratiques de l'industrie pharmaceutique freinant le développement des génériques devraient être aussi encadrés : visites médicales et cadeaux aux médecins régulés, «mee too» remboursés au prix des génériques, «pay for delay» interdit, création d'une base de données répertoriant l'ensemble des dates d'expiration de brevet. Conclusion : Cette étude montre une grande disparité de l'utilisation des génériques selon les pays et des facteurs socio-économiques. L'utilisation des génériques permet de diminuer les dépenses de santé et facilite l'accès des médicaments aux patients. Cependant cette étude manquait de données concernant certains pays (Russie, Afrique, Moyen Orient...) et de données plus récentes (période 2016-2019).
