Introduction : la loi Léonetti impose que les décisions de LATA s'appuient sur une décision collégiale. La loi prévoit également que l'avis du patient soit pris en compte. S'il n'est pas en mesure de s'exprimer lui-même, ses directives anticipées doivent être recherchées, au besoin en contactant le médecin traitant. Les services de réanimation sont fréquemment confrontés à la question des LATA. Cette étude visait donc à établir un état des lieux des échanges entre réanimateurs et MG autour des décisions de LATA. Méthodes : étude exploratoire réalisée au sein du service de médecine intensive et réanimation du CHU d'Angers. Le recueil de données a été effectué à partir des dossiers médicaux de réanimation et via un questionnaire téléphonique adressés aux MG des patients admis sur la période concernée. Les données ont été collectées dans une base de données Excel. Les tests statistiques ont été réalisés grâce au test exact de Fisher via le logiciel biostaTGV. Résultats : 103 patients ont été hospitalisés et 96% des MG figurants aux dossiers médicaux ont pu être contactés. Pour 9,7% des patients, il existait une trace écrite d'échanges entre réanimateur et MG dans les dossiers médicaux. Les MG déclaraient avoir été contactés par les réanimateurs dans 31% des cas. Les discussions de LATA ont concerné 31,1% des patients dont 71,9% aboutissaient à une décision de LATA. Par ailleurs, 40,6% des MG des patients concernés par une discussion de LATA, déclaraient avoir déjà abordé le sujet avec le patient (ou son entourage). L'étude a montré de manière significative que plus le patient (ou son entourage) avait parlé de LATA avec le MG au préalable, plus une LATA était posée lors d'une hospitalisation en réanimation (p=0,00015). Discussion et conclusion : La littérature montre que les relations MG/réanimateurs pourraient être améliorées par une meilleure communication des 2 protagonistes. Un échange interdisciplinaire existe donc bien, notamment autour des décisions de LATA, mais semble insuffisant. Notre étude suggérait que le MG pourrait être un intervenant à privilégier pour les décisions collégiales de LATA, à la fois comme détenteur d'informations signifiantes pour le patient et comme tiers extérieur à la décision.
