Introduction Dengue fever is a mosquito-transmitted disease of man which has afflicted untold millions of people over the past two centuries. It is caused by viruses classified as a subgroup of the group B togaviruses. Along with other members of that group as well as group A, the dengue viruses have been investigated intensively during recent years. Certain unique aspects of their structure, composition, antigenicity, replication, and antigenic relationships have established the togavirus family as quite distinct from other families of enveloped RNA viruses (see recent review of PFEFFERKORN and SHAPIRO, 1974). The basic studies leading to this conclusion have coincided with epidemiological field investigations which have resulted in a continuing increase in the number of viruses now designated as group A or B togaviruses. This, in turn, has led to a growing appreciation of their immense importance as actual or potential pathogens of man and beast.

Dengue is an endemic viral disease affecting human population in tropical and subtropical regions around the world, predominantly in urban and semi-urban areas. Dengue fever (DF) and its more serious forms, the dengue haemorrhagic fever (DHF) and dengue shock syndrome (DSS) are becoming important public health problems. The global prevalence of dengue has grown dramatically in recent decades. An estimated 2.5 billion people in more than 100 countries are at risk of acquiring dengue viral infection with more than 50 million new infections being projected annually, between 20,000 and 25,000 deaths, mainly in children. This book aims to review the possible molecular mechanisms that attribute to the variation of Dengue virus (DENV) disease. Additionally, a description of the animal models that have been developed to study DENV pathogenesis and to test potential vaccines and therapeutics are reviewed. This book also reviews the current understanding of DENV pathogenesis and the challenges confronting vaccine and therapeutic developers. The book also responds to past failures to control dengue in these countries, citing almost exclusive concentration on space spraying of adult mosquitoes as one of the most important causes. With these problems in mind, the book places major emphasis on measures that can make communities responsible for vector control in the domestic environment as the best way to secure sustained improvements in prevention and control. The book has twelve chapters. The first summarizes what is known about the virus, vector, host, transmission cycle, and virus-host interactions. A brief overview of the global situation is followed by a detailed chronology of the resurgence of dengue in South-East Asia. Clinical manifestations and diagnosis are covered in chapter two, which includes precise criteria for reaching a diagnosis based on clinical manifestations and laboratory findings. Chapter three, on clinical management, gives particular attention to the management of dengue shock syndrome as a medical emergency. The important role of laboratory diagnosis is addressed in chapter four, which describes in detail the laboratory confirmatory tests that are essential for diagnosis.
