The purpose of this descriptive study was to examine students' perceived confidence to provide culturally competent care. The literature showed a lack of empirical studies investigating the confidence of nurses and nursing students in providing culturally competent care. Therefore, the research question was, "What are Southwest Missouri senior baccalaureate student nurses' self-perceptions of their competence in providing cultural care for diverse ethnic clients?" A convenience sample of baccalaureate nursing students, enrolled during the fall 2009 semester, from two universities in Southwestern Missouri were recruited for this study. Students completed the Bernal and Froman's Cultural Self-efficacy Scale (CSES). This study found that the participants' self-perceived cultural knowledge, concepts, patterns, and transcultural nursing skills ranged between 3 (neutral) and 4 (moderate confidence) on the Likert scale 1 to 5. Based on the computed subscales of the CSES, the score of the total self-efficacy was 194.53, which means the participants rated themselves as neutral to moderately competent. The study found that there is a minimal increase in the self-perceived confidence levels of cultural competence compared to other studies in the Midwest.

Cultural competence is a component of the 2008 Essentials of Baccalaureate Nursing Education. Nationally, graduation exit testing has found that students continue to score as culturally aware, rather than culturally competent. This twelve article review of research literature identified curricular approaches and teaching strategies that have been found to create a student's desire to provide and demonstrate cultural competent in practice. Findings supported the continuance of a thread of culture content within all nursing theory and clinical courses through inclusion of local or international, live or simulated service-learning. To facilitate contemplation of their progression journey of cultural awareness to competence, students should maintain a cultural activities self-reflection journal at all levels to support graduates as they transition into their professional practice.

Cultural competence has been investigated extensively in nursing education to better prepare nurses to care for individuals of diverse backgrounds. While developing cultural competence is important to address individual care needs, it may not address the social and structural factors that influence health. Structural competency describes the ability to assess not only cultural factors that affect health disparities, but also structural inequalities and stigmas that can alter a patient's presenting symptoms and risk for certain diseases.
