Pursuant to a congressional request, GAO reviewed the role of school-based health centers (SBHC) in expanding children's access to health care and the financial and other obstacles SBHC must overcome to launch and maintain their services. GAO found that: (1) SBHC afford children easier access to needed health services by bringing providers to the children, furnishing free or low-cost services, and supplying the atmosphere of trust and confidentiality adolescents need; (2) SBHC do not provide all health services required by students and cannot reach adolescents who have dropped out of school; (3) a lack of stable financing is a major concern for SBHC, with some centers reporting insufficient funds to meet all children's service needs; (4) SBHC often have difficulty obtaining reimbursement from public and private insurers, including Medicaid; (5) SBHC have difficulty recruiting and retaining appropriately trained nurse practitioners and physician assistants; (6) community debates over the appropriateness of providing reproductive health services in SBHC have limited the centers' ability to meet some adolescents' health needs; (7) communities lack access to information on establishing new centers and solving problems at existing SBHC; (8) research measuring the impact of SBHC on students' health and education is sparse; and (9) although efforts to coordinate school health programs within the federal government have begun, the Department of Health and Human Services does not have a focal point to answer outside inquiries, provide technical assistance, or develop a research agenda.

The extensive education reforms that state and national policymakers have instituted in recent years have tended to overlook the reality that poor health can substantially undermine students' academic success, especially for students from low-income households. Although children in low-income communities on average have greater health needs, they are much less likely to have access to regular medical, mental health, and oral health care. School-based health centers (SBHCs) can help solve this problem. Evaluations of SBHC programs conducted since the model was implemented 40 years ago have demonstrated that SBHCs increase access to care and improve both health and academic outcomes. The major reason there are so few SBHCs is that the funding available to support them is both inadequate and unstable. To fill the gap between the insufficient funding that SBHCs now receive and the actual cost of operating SBHCs that can provide necessary and appropriate services to all students in a public school, a system that can guarantee sufficient school wide funding (GSSF) is needed. The financing strategy proposed is designed to ensure sufficient stable funding for existing SBHCs, promote strong growth in the number of SBHCs operating throughout the state and result in improved delivery of health services and greater school success for students from low-income households. The GSSF system maximizes federal reimbursements and private insurance payments, while reducing the proportion of overall SBHC funding that the state now provides. It will also promote full compliance with applicable federal and state laws, in a cost-effective manner. Recommended actions for the state are detailed to implement a guaranteed self sufficient school wide funding system for school-based health centers. They include the following: (1) provision of an adequate annual per-student rate; (2) expansion of services covered; (3) maximization of federal and private insurance reimbursements; (4) acquiring necessary federal waivers; and (5) establishment of a GSSF demonstration project. Adoption of the GSSF system would require a number of changes in state laws and regulations and a number of Medicaid waivers and legal changes. Research indicates that students living in poverty are not receiving the recommended health care needed to grow into healthy adults. School-based health centers (SBHC) have been found to fill this gap in health care, thereby improving poor students' health. Many schools, however, lack SBHCs and may even lack full time school nurses. The purpose of this project was to determine the impact of a school nurse-led educational training to classroom teachers regarding symptoms and management of diabetes on the rate of adverse diabetic incidents reported by students. A secondary purpose was to determine the impact of a nurse-led educational presentation on faculty and school administrators' knowledge regarding the benefits of school based health centers (SBHCs) with the goal of encouraging the establishment of SBHCs. The foundation for this study was grounded in Rosenstock's health belief model and Orem's self care theory. Among the four diabetic students in two classrooms, the number of reported diabetic incidents dropped from 1.9 per student per month to 1.1 per student per month, although this difference was not statistically significant. Faculty and administrators from two schools in a district serving low-SES students in Northeastern Louisiana attended a presentation on the benefits of SBHCs on student health and academics. Forty-three participants completed a questionnaire prior to the training and a week following the training. The training was found to significantly increase the intent to institute an SBHC and increase knowledge of the health and academic benefits of SBHCs. Nurses have a role to play in encouraging the establishment of SBHCs and, thereby, improving student health outcomes.
