Pourtant, n'importe quel foyer infectieux endo buccal peut avoir des répercussions systémiques que ce soit au niveau cardiaque, pleural ou cérébral. C'est le concept d'infection focale. La santé bucco dentaire est essentielle pour l'état général et la qualité de vie. Elle résulte d'un équilibre de la flore saprophyte orale. Lors d'une pathologie cérébrale, il arrive qu'après un bilan microbiologique on retrouve des bactéries issues de cette flore bactérienne, ce qui signe l'étiologie, donc la porte d'entrée. L'apport de nouvelles approches telles que la spectroscopie à résonnance magnétique permet de rapidement poser le diagnostic d'une pathologie cérébrale. Même si au cours des dernières années le taux de mortalité a baissé de plus de la moitié, et les séquelles ont quasi disparues, il est nécessaire de rappeler l'importance du rôle du chirurgien dentiste lors d'un examen clinique afin de prévenir et éliminer tout foyer infectieux pouvant être à l'origine d'une infection à distance.

Durant ces dernières décennies, les technologies médicales n'ont cessé d'évoluer permettant ainsi la survie d'un plus grand nombre de nouveau-nés prématurés. Cette évolution souhaitable augmente néanmoins l'incidence des pathologies neurologiques telle que la paralysie cérébrale par exemple. Cette dernière est un ensemble de troubles permanents du mouvement et de la posture entraînant une limitation d'activité. Elle fait suite à des lésions non progressives sur le cerveau en développement. Elle fait partie d'un cadre nosologique large comprenant notamment l'hémiparésie. De nombreuses études ont été menées sur les étiologies, les formes cliniques, les répercussions sur les systèmes d'organes et sur les membres supérieurs et inférieurs. En revanche, peu d'articles apparaissent dans la littérature concernant l'état bucco-dentaire des enfants atteints de paralysie cérébrale et d'hémiparésie. Pourtant, les problèmes neuromusculaires inhérents à ce groupe de pathologies peuvent affecter de manière significative la santé orale. De façon directe, au travers des changements qu'ils entraînent dans les structures oro-faciales ou indirecte, par le développement de para-fonctions, les problèmes d'alimentation ainsi que les difficultés de réalisation des techniques d'hygiène. Le but de ce travail est de comprendre comment et dans quelle mesure la paralysie cérébrale de type hémiparésie affecte-t-elle la santé orale des enfants atteints. Et comment le chirurgien -dentiste peut-il participer à la préservation de la santé bucco-dentaire de ces enfants qui ont un quotidien déjà compliqué.
