J. DE GROOTE One of the most ominous and troublesome complications of the liver disease is the appearance of hemorrhagic phenomena. Many careful clini cal observations about the relationship of liver function and of bilia ry tree pathology have been published. A vast amount of research work has been devoted to the subject. The severity of the hemorrhagic disor der is usually in relation to the liver disease. In mild chronic hepa titis or short lasting obstruction slight subcutaneous or mucosal blee ding may (lraw the attention of the patient and the doctor, but they are as such far from dangerous. However in acute hepatic insufficiency, in biliary cirrhosis the bleeding tendency is to be considered as a life threatening complication in about half of the cases. Moreover coagulation disturbances aggravate bleeding not only from ruptured oesophageal or gastric varices but also from gastritis or peptic ulcer. 11enometrorrhagia, epistaxis and gingival bleeding may be very trouble some in these conditions. The use of diagnostic procedures sucl. as liver puncture biopsy and peritoneoscopy are often impossible when platelets and prothrombine time are too low. In order to overcome this difficul ty a procedure has been worked out taking a biopsy through a trans jugular catheter placed in the hepatic vein. If a bleeding from the liver occurs it will be in the circulatory system and not cause any trouble.

Hypercoagulability and bleeding are problems physicians frequently encounter when treating intensive care patients. The coagulation mechanism is a tightly controlled system. Its balance is disturbed in conditions such as septicemia, trauma, shock, and others. The dis turbances may be local or disseminated and the therapeutic options available are limited and nonspecific: limited since in most cir cumstances they may not affect the underlying disease and are si milarly independent of the pathogenic mechanism; nonspecific since they cannot be specifically targeted to the problem of local or sys temic disturbance of coagulation. Furthermore, increased and de creased coagulability may be present at the same time. Activation of coagulation may lead to fibrin deposition and decreased hemostatic capacity, as seen in disseminated intravascular coagulation. New diagnostic approaches for acquired disorders of coagulation have been developed and form a new basis for therapeutic strategies. Despite this progress, many physicians are aware of the problem that, especially for intensive care patients, a treatment very often has to be selected before diagnostic procedures can be completed. We also know that for many diseases rare occurrence or the hetero genity of the patient population make it quite difficult to conduct clinical studies which prove that a therapeutic regimen is efficacious. Thus, much discussion about coagulation disorders in critically ill patients is still necessary. Even so, it is important to provide guidelines for handling these problems.

The understanding of hemostasis physiology has been considerably advanced by models of kinetics and by the complicated interplay of cells and soluble coagulation factors. How this physiology is currently, or will eventually, be reflected in clinical laboratory testing is the subject of this monograph; this information is key for laboratorians to implement physiologic concepts into practical data and for clinicians to understand the basis of and therefore correctly treat hemostatic disease. Many thousands of patients present with bleeding disorders or bleeding complications of other diseases, and millions of individuals suffer the morbidity and mortality of thromboembolic complications. How laboratory testing confronts the challenge of predicting different hemostatic risks and guiding therapy is the critical subtext of the chapters in this monograph. New developments in hemostasis physiology have identified thrombin as an important, if not central, coordinator of hemostatic function and thus a target for measurement to assess hemostatic function and risk. Whether such measurements as endogenous thrombin potential or thromboelastography will accurately predict and/or quantitate global hemostatic function is an important question. Modeling the clinical risks of bleeding or thromboembolism currently uses the laboratory presence or absence of particular risk factors, but our clinical understanding of risk appears to more closely approximate a dynamic model, even within individual patients. Therefore, testing for platelet dysfunction or comprehending the functional implications of a lupus anticoagulant may rely on our evolving comprehension of hemostasis phyisology and perhaps require more sophisticated interpretation of clinical predictors of hemostatic risk. This monograph aims to shed some light in these areas and promote investigation of such key hemostasis issues.
