La prise en charge du cancer a été marquée ces dernières années par le développement des chimiothérapies ciblées dont le mode d'action est basé sur les mécanismes mêmes de l'oncogenèse avec une spécificité importante pour les cellules cancéreuses. Une partie de ces molécules est disponible par voie orale avec pour conséquence une modification de la prise en charge des patients au profit du secteur ambulatoire. Actuellement, 77 anticancéreux oraux disposent d'une Autorisation de Mise sur le Marché (AMM). L'AMM a vocation à valider l'évaluation scientifique de chaque médicament pour assurer que le bénéfice pour les patients l'emporte sur les risques associés à la consommation de la molécule. Elle précise les conditions d'utilisation pour lesquelles l'efficacité et l'innocuité de la molécule ont été validées. Les médecins sont parfois amenés à prescrire en dehors de l'AMM des médicaments, on parle de prescription hors-AMM. Ces pratiques sont connues en cancérologie (situation clinique pour lesquels aucune molécule n'a l'AMM, populations particulières avec manque de données cliniques disponibles). A l'hôpital, le suivi du hors-AMM en cancérologie est particulièrement encadré. Les chimiothérapies orales, parce qu'elles sont disponibles en ville échappent à ce suivi. L'objectif de cette étude, menée sur un recueil de patients du Centre Henri Becquerel, était d'évaluer la prévalence des prescriptions hors-AMM de chimiothérapies orales et leur pertinence à travers l'analyse des justifications bibliographiques. Si la majorité des prescriptions hors-AMM repose sur des justifications bibliographiques à haut niveau de preuve, cette étude met en évidence un manque de traçabilité de ces justifications dans les dossiers des patients. Il semble nécessaire de sensibiliser les prescripteurs à l'importance de tracer leur argumentation en cas de recours aux prescriptions hors-AMM. Par ailleurs, cette étude met l'accent sur la nécessité d'optimiser le système d'identification des patients sous chimiothérapie orale au CHB.

Un nombre croissant de patients atteint par le cancer bénéficie d'une thérapie ciblée avec comme modalité d'administration la voie orale, du fait de son efficacité et de l'apport en termes de qualité et de confort de vie. Cependant, ces cytotoxiques ne doivent pas être banalisés car ils présentent des effets indésirables, de nombreuses interactions médicamenteuses et alimentaires, et imposent des plans de prises parfois complexe. Ils suscitent de nouvelles interrogations concernant suivi et tolérance, dispensation, observance et implication du pharmacien d'officine dans la prise en charge des patients concernés. L'objectif de cette étude était d'initier au sein d'un centre hospitalier général des entretiens pharmaceutiques concernant l’ensemble des chimiothérapies orales avec la création d'un lien Hôpital-Ville. La première partie de ce travail présente le contexte en axant sur l’épidémiologie du cancer et les différents plans cancer. Dans un second temps, un descriptif des traitements anticancéreux oraux est réalisé ainsi que la présentation de leurs avantages et inconvénients. Puis les spécificités de la validation pharmaceutique à l’officine des prescriptions de chimiothérapies per os sont évoquées. Enfin, la mise en place d’entretiens pharmaceutiques à l’Hôpital Nord-Ouest puis officinaux, est exposée. L'évaluation des patients après les deux entretiens montre une réelle progression des compétences acquises. Les résultats obtenus permettent d'aborder le rôle et la formation du pharmacien d'officine sur ce thème, ainsi que l'importance de la mise en place d'une coordination Hôpital-Ville dans le parcours de soin transversal des patients
