Problématique : Les soins palliatifs sont des soins délivrés par une équipe interdisciplinaire, dans une approche holistique, afin d'améliorer la qualité de vie des personnes atteintes d'une maladie grave et potentiellement mortelle. Selon le World Health Organization et la Worldwide Palliative Care Alliance (2014), des inégalités dans les formations et le manque d'organisation des soins palliatifs dans plusieurs pays n'ont pas favorisé le développement de la pratique infirmière dans ce domaine. Ainsi, cette situation a entraîné des disparités dans la prestation et les services de soins palliatifs entre différents pays. Au Liban, les infirmières sont peu préparées à délivrer des soins palliatifs et la pratique dans ce domaine est peu documentée. But : Cette étude visait à comprendre l'exercice de la pratique infirmière en soins palliatifs dans une région du Liban, auprès de patients atteints de cancer en fin de vie et au sein d'une équipe interdisciplinaire. Cadre de référence : Le cadre de référence de l'étude intègre trois éléments, soit le plan directeur de développement des compétences des intervenants en soins palliatifs (MSSS, 2008), les facteurs contraignant ou facilitant la pratique infirmière en soins palliatifs, ainsi que la théorie du Human Caring de Jean Watson (1979). Méthode : Dans le cadre de cette recherche, le devis descriptif qualitatif choisi est une étude de cas simple à niveaux d'analyse imbriqués. Cette étude est réalisée dans une unité d'oncologie d'un centre hospitalier, dans une région urbaine du Liban. L'analyse des données a été effectuée par une double triangulation de méthodes et de sources, soit l'entrevue individuelle auprès de l'infirmière spécialisée en soins palliatifs (n= 1), auprès de l'infirmière-chef en oncologie (n= 1), auprès des infirmières de l'unité d'oncologie (n= 9), auprès des experts en soins palliatifs (n=3), auprès de certains membres de famille de personnes soignées (n=3) et l'entrevue de groupe auprès des professionnels de la santé de l'équipe de soutien mobile (n=3), l'étude des documents pertinents, l'observation directe non participante de la pratique infirmière en soins palliatifs et le journal de bord de l'étudiante-chercheuse. Résultats : Cinq thèmes centraux ont émergé de la pratique infirmière auprès de patients cancéreux au sein d'une équipe interdisciplinaire au Liban : 1) la perception des soins palliatifs : un moyen d'offrir une meilleure qualité de vie; 2) la pratique infirmière : une prise en charge globale des besoins du patient; 3) la pratique en collaboration interdisciplinaire : un élément clé pour les soins palliatifs; 4) la spiritualité : une toile de fond pour la pratique infirmière en soins palliatifs et 5) l'accompagnement de la famille : le rôle indispensable de l'infirmière. De plus, les résultats ont mis en relief divers facteurs, à la fois contraignant ou facilitant la démarche. Ces facteurs sont d'ordres professionnel, organisationnel et émotionnel. Ils sont associés à l'exigence de la pratique en soins palliatifs. Les infirmières ont démontré également une relation humaniste empreinte de caring au cœur des soins palliatifs et qui transcende l'ensemble des cinq thèmes centraux. Discussion : Cette étude a permis de mettre en lumière l'exercice de la pratique infirmière en soins palliatifs, au sein d'une équipe interdisciplinaire. Les résultats de cette étude pourront fournir des assises empiriques afin de guider le développement de la pratique infirmière en soins palliatifs au Liban et assurer une meilleure prise en charge en fin de vie.
