Considering the detrimental environmental impact of current food systems, and the concerns raised about their sustainability, there is an urgent need to promote diets that are healthy and have low environmental impacts. These diets also need to be socio-culturally acceptable and economically accessible for all. Acknowledging the existence of diverging views on the concepts of sustainable diets and healthy diets, countries have requested guidance from the Food and Agriculture Organization of the United Nations (FAO) and the World Health Organization (WHO) on what constitutes sustainable healthy diets. These guiding principles take a holistic approach to diets; they consider international nutrition recommendations; the environmental cost of food production and consumption; and the adaptability to local social, cultural and economic contexts. In December 2016, FAO and WHO convened an International Symposium on Sustainable Food Systems for Healthy Diets and Improved Nutrition, gathering delegates from 90 UN Member States representatives of intergovernmental organizations, private-sector entities, civil society organizations, academia/research organizations and producer organizations/cooperatives. The symposium aimed to increase awareness of today’s urgent food and nutrition challenges, and to create a forum to discuss strategies for regulation and reform, in the aftermath of the ICN2 and under the umbrella of the UN Decade of Action on Nutrition 2016-2025. Nine parallel sessions comprising expert presentations and country case studies were complemented by a session on the United Nations Decade of Action on Nutrition, a student’s session, plenary and special events. The national food system of Bangladesh has made substantial progress since experiencing famine in 1974, soon after independence. After the famine, the government placed a strong emphasis on policies required to attain grain self-sufficiency; since attaining self-sufficiency, the production system, policies related to it, and resulting diets have begun to diversify. Nonetheless, undernutrition remains a problem, and fruit and vegetable consumption are inadequate for most people relative to international recommendations. Moreover, as the food system has begun to transition towards a modern one, challenges related to food safety and perceived food adulteration have begun to rise. Further, increased processed food intakes are potentially associated with existing rising overweight and obesity status. Both government interventions and innovations are needed to help shift the national food system to improve nutrient-dense food availability, particularly among the poor, and to limit the increase in processed food consumption.

In many respects, the continent of Africa is in transition. Prominent among them – currently – is the nutrition transition. One consequence of the nutrition transition is the increase in prevalence of nutrition-related non-communicable diseases (NCDs) such as obesity, type 2 diabetes, cardiovascular diseases, and certain cancers. Although NCDs are a global public health problem, the rate of increase in NCDs morbidity and mortality in some African countries is staggering. This surge has been linked to modifiable environmental factors – factors that facilitate the consumption of obesogenic (energy-dense nutrient-poor foods), rather than unrefined cereals, fruits, and vegetables. It has long been recognized that the physical and social environments - in which people live, work, and eat are critical determinants of their health. More recently, there has been a greater focus on the food environment as a key determinant of health. Available evidence shows that unhealthy food environments drive unhealthy diets; and unhealthy diet is one of four main risk factors for NCDs.
