From more than 7,000 patients studied with abdominal CT, we tried to assemble images with maximal anatomical detail, which implies the use of large window settings, being well aware that in daily routine practice basic CT methodology includes the use of different window settings for optimal information. It was, however, a conscious decision not to use comparative images with large and small window settings in order to stay within a reasonable total number of figures. Much emphasis has been placed upon the use of contrast enhancement by intravenous contrast media. This is based on the conviction that essen tially new and better morphological information about normal and pathological processes within the abdomen can be obtained because short exposure times now allow one to capture the rapidly changing aspect and degree of contrast enhancement of the lesions as a function of their vascu larity.

The purpose of this book is to provide the radiologist with information which is "as practical as possible" for the everyday use of computerized tomography (CT) in the field of cervical, thoracic, and musculoskeletal pathology. The approach is simple. For each region the following information is pre sented: (1) a general schematic introduction, summarizing the main indications for CT and its specific usefulness; (2) a series of pictures of normal structures with a precise and practical identification; and (3) a selection of pictures of pathological structures, with a description and a short comment, aimed at covering the largest possible field of CT indications and interests. Renal sonography forms a basic part of routine diagnostic strategy. This textbook summarizes eighteen years of experience in diagnostic ultrasound. We want it to serve as a guide to both imagers and mere consumers of information. That is why we shall emphasize practical advice and diagnostic pitfalls; it is also why we shall often deal with the relations of sonography with other diagnostic procedures, which it may complement or replace, precede or follow, the purpose being to achieve efficiency at low cost. We shall limit our subject matter to the kidney itself and the neighboring retroperitoneal compartments, dealing only briefly with the lower urinary tract, which requires specialized procedures. We devoted considerable space to renal sonography in our book Clinical Atlas of Ultrasonic Radiography, published in 1973. Since then, nothing has changed and everything has changed. Nothing, because even then the differential diagnosis between a solid and a cystic mass, the etiologic diagnosis of a nonsecreting kidney, and the positive diagnosis of a traumatic juxtarenal hematoma were quite reliable, making possible drastic reductions in the indications for instrumental roentgenologic examinations. Everything, be cause improved resolution and grey scale imaging (already partially achieved in 1973, thanks to real time) have profoundly refined both anatomic and pathologic ultrasonic studies. And now high resolution real time imaging has revolutionized renal examination techniques, whereas Doppler is entering routine ultrasonic diagnosis.

Progress in traumatology of the vertebral spine has been restrained for a long time by two hindering factors. The first obstacle is presented by the differences in approach and a conflict of competences. The neurosurgeons, considering only the spinal chord, have confined themselves to indications for laminectomy, an insufficient and usually ineffective intervention. The orthopedic surgeons, on the other hand, obsessed by the fear of medullary lesions, have long hesitated to apply the fundamental rules for the treatment of fractures, namely precise reduction followed by strict immobilization, thus depriving themselves of the efficacy of radiculomedul lar decompression and of the protection this procedure affords to these structures when they are themselves involved in the trauma. Taking these facts into account, together with the rather poor results of laminectomy, the specialists have wisely and successfully recommended that one should abstain from treating the initial lesion, but rather attenuate the damage by appropiate nursing care and adequate reeducation. The second hindering factor was the insufficient knowledge of the extreme complexity of the anatomic lesions. This explains the orthopedists' relative caution; one only treats well what one knows well. More precise analysis of the lesions, not only of the bones but also of the joints, i. e., the osteofibrous involvement, is mainly based on strict radiologic semiology, which is rendered difficult because these structures are simultaneously affected. We are greatly indebted to Dr. DOSCH, radiologist at our Center, for having untertaken this difficult task, using all available conventional radiologic techniques.
