L’objet de cette thèse est d’analyser l’action de dépistage des facteurs de risque cardiovasculaire menée par le réseau "Cœur en Santé". Le but de ce dernier est de repérer précocement des patients à risque cardiovasculaire non connus dans le but d’améliorer leur prise en charge. De septembre à mai, environ deux fois par semaine, les individus qui le souhaitent se présentent au dépistage ; celui-ci a lieu dans un site différent à chaque fois afin de recouvrir les 24 communes qui constituent le champ d’action du réseau. Les participants rencontrent successivement la diététicienne puis l’infirmier. Ce dernier dépiste, entre autres, les diabétiques et effectue, lorsque cela est nécessaire, des clichés du fond d’œil grâce au rétinographe non mydriatique pour rechercher une atteinte rétinienne. Il identifie également les autres facteurs de risque cardiovasculaire avant de proposer une prise en charge adaptée (ateliers-cuisine, ateliers sur l’équilibre alimentaire et marches encadrées). Au total, 2841 patients se sont présentés au réseau. Il s'agit d'une population précarisée (23,6% de chômeurs, 10,3% de CMU) et marquée par des facteurs de risque cardiovasculaire (26,1% d’obèses, 15% de diabétiques connus ou dépistés, 33,1% de tabagiques actifs ou sevrés, 30% d'hypertendus). L'analyse du dépistage porte sur la motivation des participants, la rentabilité et les apports du réseau "Cœur en Santé". Il apparaît notamment que la participation est motivée par l'inquiétude des patients sur leur état de santé. Il ressort également que le paramètre qui discrimine le plus efficacement notre population est le tour de taille. Pour finir, nous notons que le réseau "Cœur en Santé" et les réseaux diabète en général appartiennent à un dispositif visant à dépister et prendre en charge plus vite et plus tôt. Leur rôle dans le parcours de soins prendra probablement une nouvelle ampleur à l'occasion des programmes de "disease management" prévus par l'Assurance Maladie pour 2008
