This report describes recent trends in the international migration of doctors and nurses in OECD countries. Over the past decade, the number of doctors and nurses has increased in many OECD countries, and foreign-born and foreign-trained doctors and nurses have contributed to a significant extent. New in-depth analysis of the internationalisation of medical education shows that in some countries (e.g. Israel, Norway, Sweden and the United States) a large and growing number of foreign-trained doctors are people born in these countries who obtained their first medical degree abroad before coming back. The report includes four case studies on the internationalisation of medical education in Europe (France, Ireland, Poland and Romania) as well as a case study on the integration of foreign-trained doctors in Canada.

Migration from and within Africa, just like migration elsewhere in the world, often generates anti-immigrant sentiment and ignites heated public debate about the migration policies of the destination countries. These countries include South Africa as well as others outside the continent. The countries of origin are also keen to minimize losses through "brain drain" and to capture resources such as remittances. Increasingly, international organizations and human rights advocates have stressed the need to protect the interests of migrants themselves. However, while the UNDP's 2009 Human Development Report talks of "win-win-win" solutions, in practice it is the perceived interests of destination countries that enjoy the greatest attention, while the rights of migrants themselves are afforded the least. Yet migration is not just an issue in itself: it also points to structural inequalities between countries and regions. Managing migration and protecting migrants is too limited an agenda. Activists and policymakers must also address these inequalities directly to ensure that people can pursue their fundamental human rights whether they move or stay. It is not enough to measure development only in terms of progress at the national level: development must also be measured in terms of reductions in the gross levels of inequality that now determine differential rights on the basis of accident of birth.

Privatizing Health Services in Africa analyzes the disappearance of public health in the form of state services in Africa, and the growth of a private market in health care that will serve primarily an urban elite. Meredeth Turshen considers the implications of introducing private insurance in countries with growing unemployment, a shrinking formal job sector, and a lack of social security programs or other safety nets. She debates the pros and cons of shifting the delivery of health services to the nongovernmental sector in the context of new concepts of the role of the state. Many of the schemes to privatize the purchase and sale of pharmaceuticals reverse decades of United Nations work challenging the power of the multinational drug industry. Turshen weighs these policy changes in light of the World Bank's eclipse of the World Health Organization as the premier UN health policy agency. Until now, no book has disputed the World Bank's plans to privatize health care in Africa. This is the first book-length analysis of policy changes in light of monetarism and globalization. The 2006 World Health Report focuses on the chronic shortages of doctors, midwives, nurses and other health care support workers in the poorest countries of the world where they are most needed. This is particularly true in sub-Saharan Africa, which has only four in every hundred global health workers but has a quarter of the global burden of disease, and less than one per cent of the world's financial resources. Poor working conditions, high rates of attrition due to illness and migration, and education systems that are unable to pick up the slack reflect the depth of the challenges in these crisis countries. This report considers the challenges involved and sets out a 10-year action plan designed to tackle the crisis over the next ten years, by which countries can strengthen their health system by building their health workforces and institutional capacity with the support of global partners.
