Introduction. Les anticoagulants sont l'une des classes médicamenteuses les plus fréquemment responsable d'accidents iatrogènes comme les saignements. Malheureusement, très peu de données sont disponibles concernant les caractéristiques et la gestion des patients admis en réanimation pour un saignement grave sous anticoagulant. Méthodologie. Une étude observationnelle rétrospective a été menée dans 4 services de réanimation de CHU français. De janvier 2007 à décembre 2018, tous les patients âgés de plus de 18 ans, admis en réanimation pour saignement extra-cérébral lié à l'anticoagulation, ont été inclus. Résultats. Au total, 393 patients ont été inclus dans les 4 unités françaises de réanimation. L'âge moyen était de 73 ± 13 ans, 38 % étaient des femmes et la plupart des patients présentaient des comorbidités (hypertension artérielle 70 %, cardiopathies 58 %, diabète 36 %). Les anticoagulants responsables du saignement étaient les anti-vitamines K (AVK : 53 %), les héparines (25 %) et les anticoagulants oraux directs (AOD : 7 %). L'incidence des saignements sous anticoagulants a augmenté au cours de la période 2007-2018, passant de 3.7/1000 admissions en 2007 à 5.5/1000 admissions en 2018, avec une stabilité de la responsabilité des AVK et une augmentation des saignements sous AOD. La mortalité en réanimation était d'environ 30%. En analyse multivariée, les facteurs associés à la mortalité étaient : Glasgow 12, TCA1.2, hyperlactatémie et support ventilatoire. La gestion médicale des hémorragies liées à l'anticoagulation et notamment l'antagonisation était peu conforme aux recommandations françaises et internationales, avec l'utilisation des concentrés de vitamine K et de complexe prothrombique perfusés uniquement chez 60 % des patients traités par AVK, et l'utilisation des concentrés de complexe prothrombique chez 25 % des patients traités par AOD. Conclusion. Les hémorragies extra-cérébrales sous anticoagulant admises en réanimation sont des complications graves responsables de défaillances d'organes et d'une mortalité importante. Leur incidence est en augmentation. La formation des cliniciens pour la prise en charge thérapeutique de ces accidents doit être améliorée.

They are crucial for hemostasis, and yet excessive platelet activation is a major cause of m- bidity and mortality in western societies. It is therefore not surprising that platelets have become one of the most extensively investigated biological cell types. We are, however, far from understanding precisely how platelets become activated under physiological and pathophysiological conditions.
