Not everyone is a friend of the manifold abbreviations that have by now beCome a part of the scientific language of medicine. In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.

Introduction : Le traitement anticoagulant à dose curative améliore le pronostic de nombreuses pathologies vasculaires thrombo-emboliques mais peut se compliquer d’hémorragies. Notre objectif est d’étudier les accidents hémorragiques associés aux anticoagulants (AHAA) nécessitant une prise en charge en réanimation. Méthodes : Etude monocentrique observationnelle rétrospective menée dans un service de réanimation médicale de 18 lits entre Janvier 2000 et Décembre 2013. Ont été inclus les patients sous anticoagulation efficace dont le motif d’hospitalisation en réanimation était lié à un AHAA. Les hémorragies intra-cérébrales ont été exclues. Résultats : Pendant cette période de 14 ans, 12214 patients ont été admis. 122 AHAA ont été identifiés et 100 AHAA analysés. Ont été exclus 3 cas en raison de données manquantes, 19 cas pour AHAA intra-cérébral. L’âge moyen était de 7711 ans avec 62% de femmes. Les saignements étaient localisés dans le thorax (6%), le tube digestif (31%), le rétropéritoine (29%), la paroi abdominale (22%) et d’autres sites (12%). Les principales indications de l’anticoagulation étaient la fibrillation auriculaire (46%) et la maladie thrombo-embolique veineuse (38%). La mortalité globale en réanimation était de 30%, elle était significativement plus faible lors d’une hémorragie digestive (16%, p=0,04) et plus importante en cas de localisation à la paroi abdominale (55%, p=0,004). Les AHAA de la paroi abdominale étaient principalement consécutifs à un geste traumatique et en particulier à une injection d’anticoagulant. Conclusion : Les AHAA admis en réanimation sont des évènements graves surtout lorsqu’ils surviennent au niveau de la paroi abdominale. Un certain nombre des AHAA pourraient être évités.
