Excerpt from A Handbook on Leprosy This handbook has been prepared in the hope of assisting those who have not had an opportunity of becoming well acquainted with leprosy, in recognising the disease, and appreciating its nature. The book does not claim to be anything but a practical aid to those in want of assistance. Many other writers have entered more fully and minutely into the various branches of the subject, but it has been my endeavour to place before my readers, in as concise a form as possible, all that is at present known concerning it. I have consequently purposely refrained from mentioning the various theories that have from time to time gained credence as to the cause of the disease. More especially as it is now almost universally acknow ledged that leprosy is due to the presence in the system of the bacillus leprae. I have for the same reason omitted to discuss the question of heredity, more so as I am aware that there are now very few supporters of this theory as to its causation. Excerpt from Leprosy In 1874, Dr. Hansen reported to the Medical Society of Christiania that 'he had often, indeed generally, found when seeking for them in the leprous tubercles, small rod-shaped bodies in the cells of the swellings.' This was the first announcement of the discovery of the bacillus of leprosy, which in less than ten years from that date had been found to be invariably present in leprous tissues in all countries where the disease occurs. The discovery of a specific organism naturally raised afresh the question as to the contagiousness of the malady with which it is associated; leprosy, which had at one time been treated as a most contagious disease, having come to be considered in modern times, by many medical men, as not contagious at all. With the recognition of this special organism, the supposition that leprosy might be contagious was naturally strengthened; but although of late years the belief in its communicability has been steadily growing stronger, we are yet far from having attained unanimity on the point. One of the chief objects of this book is to systematise, for the convenience of the medical profession, the knowledge that has been acquired up to the present time relating to the bacillus leprae, its relations to the pathological changes peculiar to leprosy, and its important bearings on the etiology of the disease. Not illustrated. 1896 edition. Excerpt: ... to become harder and firmer with age, but, if the varieties above referred to are recognised, the clinical history of leprosy becomes regular and certain. The fibroid degeneration of the nodules is a most interesting fact, and one which will, I believe, yet lead to the adoption of the only method by which leprosy can be cured--that is by the production of fibroid degeneration by appropriate treatment. (b) Ax-esthetic Leprosy. The symptoms of this form of leprosy are not as varied as those of the tubercular, for there are no varieties of anaesthetic leprosy, and each case is exactly like another, except in the severity of the disease, which may, indeed, become arrested in every stage of its existence. The usual prodromata are followed by the erythematous rash, as in tubercular leprosy; but the rash, unlike that of tubercular leprosy, tends towards permanency and enlargement. The spots are due to the poison entering the general circulation and acting upon the terminal branches of the cutaneous nerves, and not, I imagine, to the presence in the skin of the bacilli themselves; though, as might be supposed, these may, after the cutis has become weakened by their virus, actually invade this tissue. Of this, however, I am not prepared to speak with any authority, as I have been unable to satisfy myself on this point. The nerves are at first irritated by the poison, and the spots become sympathetic and painful, but the former are soon more or less disorganised when the latter become gradually anaesthetised. The whole thickness of the skin is affected by the loss of nerve-power, and each of its component parts becomes atrophied, but the most marked visible change in the skin is the absorption of its pigment (see Plates XXIII. and XXIV.). Excerpt from Suppression and Prevention of Leprosy The thing to which Dr. Goldschmidt and myself attached the greatest importance, was that there should be formed a general committee, composed of official delegates from all governments. Discussions of leprologists, about etiology, etc., were of secondary importance, for whatever is known about leprosy can be learned by any physician, without at tending a congress. Moreover, it is generally admitted that there is no cure for leprosy. Therefore, prevention and sup pression of leprosy can only be obtained by segregation, en forced, of course, by law, and to that effect the meeting of official delegates, not of scientists, was required. The question of the place where the congress was to meet, was, of course, of inferior importance. We naturally thought of Bergen first. As the Scandinavian government seemed to be afraid to take so much upon itself, we turned our minds to London. There we found an obstacle in the general disbelief in contagiousness. Then Moscow was pro posed; and finally, independent of Dr. Goldschmidt and my self, an effort was made and proved successful, to obtain from the German government an invitation to other governments to send their delegates to Berlin. This sending of delegates was what we had been fighting for; it is our plan exactly. Therefore, we renounce our own scheme of calling a meeting at Moscow, content to find our wishes realized, whatever may be the seat of the leprosy congress. Excerpt from Leprosy and the Charity of the Church Leprosy seems to be a very ancient disease. The 13th chapter Of the Book Of Leviticus treats of the law concerning leprosy in men and garments, and the 14th chapter, Of the rites of sacrifice in Cleansing leprosy. It was regarded by the ancients as adis ease peculiar to Egypt. Herodotus speaks of it: as existing in Persia. Pliny knew of it, as did Diodorus. Lucretius says: Leprosy is a disease born in Egypt along the waters Of the Nile, and nowhere else. The Hebrews brought it with them from their Egyptian slavery and the Mosaic law shows how dreadful must have been its ravages and how great the terror which it excited. Regarding it as a disease sent directly by God, they required that the person supposed to be infected should go and Show himself to the priest, and, if in his Opinion, the disease was leprosy, he was declared unclean and at once separated from the people.
