Two critical mishap program documents, one for the USAF and one for the Air National Guard, are reproduced here. This instruction implements Air Force Policy Directive (AFPD) 91-2, Safety Programs. It establishes mishap prevention program requirements, assigns responsibilities for program elements and contains program management information. The Air Force System Safety Handbook was prepared as a resource document for program office system safety managers and system safety engineers. It is not designed to answer every question on the topic of system safety nor is it a cookbook that guarantees success. The handbook provides considerable insight to the general principles, objectives, and requirements of applying system safety concepts to the Air Force system acquisition and logistical support processes.
