PREFACIO Los temas tratados en este libro, Trauma, cirugía de urgencia y cuidados intensivos, plantean un reto importante por muchas razones. Obviamente, sería inapropiado tratar de analizarlas en el prefacio. Me gustaría, sin embargo, alertar al lector sobre algunos aspectos básicos que reflejan el perfil del acceso a la salud ofrecido a la población de los países involucrados y las características de la atención médica proporcionada. El trauma sigue siendo un reto en todos los países de América, y particularmente en América Latina, donde los homicidios son extremadamente frecuentes y afectan, especialmente, a la población más joven. Todas las iniciativas para dar a conocer el impacto del trauma y para promover la prevención y mejorar el servicio han sido ineficaces, lo que demuestra claramente que el trauma sigue siendo visto por los funcionarios públicos y políticos como un fenómeno "accidental". Es interesante observar que, por el contrario, recursos altamente significativos han sido invertido en las áreas del cáncer y de las enfermedades cardiovasculares, enfermedades que, en la mayoría de los casos, nada más representan mecanismos fisiológicos del final de la vida. Hoy día, al visitar las unidades de cuidados intensivos, es común que las pocas camas disponibles se encuentren ocupadas por pacientes de edad avanzada, a menudo personas con enfermedades incurables y tratados en unidades con equipamiento y recursos pobres. Se incentiva la "inutilidad médica". Se invierten enormes recursos para permitir que los pacientes sobrevivan unas pocas semanas más, al costo de una calidad de vida totalmente inaceptable. El acceso a los cuidados paliativos es visto, a menudo, como una falta de respeto al paciente y su familia. Las causas más importantes de tales distorsiones inaceptables se pueden agrupar en los siguientes aspectos. Inicialmente, se debe considerar el cambio del perfil de la población. Cada año aumenta la proporción de personas mayores y, en consecuencia, se incrementa el número de pacientes con trastornos neurodegenerativos, los candidatos a la muerte por cáncer y enfermedades cardiovasculares que, a pesar de los avances de diagnóstico y terapéuticos, siguen siendo incurables.
