Adolescents must enter adult health care services at age 18, yet most are not ready and have not mastered daily self management of their type 1 diabetes. The purpose of this study was to determine the feasibility of a transition planning intervention, focused on educating adolescent/parent dyads about diabetes self management in preparation for the transition from pediatric to adult health care, using a newly developed transition plan and framework. The setting was a diabetes endocrinology clinic where 95% of young type 1 diabetics in the metropolitan area are seen. The intervention included supporting the parent in the relinquishing of diabetes management responsibilities to the adolescent. This study measured the transition readiness and self management practices of the adolescent as well as the parents' ability to promote autonomy in their adolescents' transition to self management. In addition, this study sought to gain information from the dyads on the usefulness of this intervention. The intervention consisted of four weekly sessions emphasizing a review of type 1 diabetes pathophysiology, daily living with type 1 diabetes, leaving the parental home, and reproductive health. A non-experimental, pre-post feasibility design was used and 11 adolescent/parent dyads consented to participate. Seven dyads completed the intervention. Using descriptive statistics, mean scores improved for transition readiness, diabetes care activities, problem solving, communication, and goals setting. Post intervention evaluations completed by participants were favorable. Most agreed or strongly agreed that the transition intervention was helpful in getting adolescents ready for transition and taking care of diabetes as a young adult. They would recommend this intervention to others. Based on the findings of this study and evidence found in the literature, the intervention will soon be packaged into brief 15-minute teaching sessions presented during quarterly clinic visits for all adolescents age 15-18 at the study site. If any adolescent remains in the clinic until age 19 or later, the intervention will continue quarterly until the adolescent transitions to adult health care. In addition, based on the responses given by the adolescent/parent dyads at completion of the sessions, this intervention contains useful information relevant to the adolescents' transition to adult health care.

Background: Care transitions, defined as hospital discharge or movement from one healthcare setting to another, are currently a major concern for healthcare providers and policy makers alike. These care transitions are occurring in the context of increasingly fragmented care and have been known to result in hospital readmissions, adverse events, and medication errors. The adolescents' transition between pediatric and the adult care system is not only fragmented but occurs at a time when they are most vulnerable and ill equipped for change. Objectives: The purpose of this research was to conduct a feasibility study to examine if the use of a diabetes transition program, SMILE (Self-Management In Living Everyday), improves glycemic control in adolescents with type I diabetes mellitus (TIDM). The aims of the study were to examine 1) pre and post scores in diabetes knowledge, diabetes distress, and glycemic among a sample of adolescents with TIDM participating in the SMILE program, 2) relationships between diabetes knowledge, diabetes distress, and glycemic control among a sample of adolescents with TIDM participating in the SMILE program, and 3) the feasibility and acceptability of an individualized transition program utilizing technology, health care advocates, and home education visits among a sample of adolescents with TIDM participating in the SMILE program. Methods: A descriptive, correlational, repeated measures design was used for this feasibility study. Both quantitative and qualitative data were collected from a purposive sample of 14 adolescents recruited from patients who received care in the endocrine clinic at a large academic children's hospital located in Southern California. Data were collected during the spring of 2012 and participants' total time in the program was three months. Study measures included The Diabetes Knowledge Test (Fitzgerald, et al., 1998), Diabetes Distress Scale (DDS17), (Polonsky et al., 2005), Hemoglobin Al c, Frequency of Self Monitored Blood Glucose Testing (SMBG), and selected demographics. Statistical analysis included descriptive and inferential statistics. Complications arising from inadequate follow up and treatment not only represent a disruption to care but also result in significant increases in the morbidity and mortality of such patients. One particular challenge faced by pediatric healthcare practitioners is the inevitable need to transfer care from the pediatric to the adult healthcare setting. Unsuccessful patient transfer results in the loss of appropriate care and an increase in the risk for adverse health outcomes. The main obstacles to the timely transfer of care are a lack of adequate patient preparedness for transfer of care and inadequate inter-physician networking/sharing of information in a culturally competent manner. Definition/Goals : Given these obstacles, this study stipulates that the provision of educational and networking resources to patients and adult care practitioners during the transition process will enable more effective transitioning from pediatric to adult care in T1DM patients. The main goal of this project is to create culturally appropriate transition tools and secondarily to identify and address obstacles to the process of educating patients and the establishment of networks among providers to the transitioning population. Methods/Materials : English and Spanish transition tools including patient and parent questionnaires, practitioner health summary tools, and transition brochure will be created. Materials will be reviewed by transition providers and clinic staff for effectiveness. Outcomes : These tools will then be made available to patients and practitioners through the Pediatric Endocrine clinic for immediate use in transitioning patients. The effectiveness of the transition tools will be assessed in follow up studies determining the percentage of transition teens scheduled and seen by their new providers and by feedback from both the transitioning population and their new providers.
