Le carcinome mammaire est le premier cancer de la femme jeune. Il suscite un intérêt particulier du fait de ses taux de mortalité et de morbidité plus importants que chez les femmes plus âgées. En effet, les patientes jeunes présentent des maladies globalement plus agressives avec une moins bonne survie et probablement plus de conséquences indésirables des traitements. Notre travail vise à une meilleure compréhension du carcinome mammaire chez cette jeune population afin d'optimiser leur prise en charge thérapeutique. À partir des données issues d'une large cohorte nationale multicentrique de patientes opérées d'un cancer du sein localisé, nous avons réalisé deux études centrées sur les femmes jeunes : Étude I : La première étude a analysé les facteurs pronostiques communs et intrinsèques d'une cohorte de 556 patientes âgées de 35 ans ou moins, par rapport à un groupe témoin de 5 925 patientes âgées de 36 à 50 ans, et a évalué le rôle de l'âge en termes de survie sans récidive (SSR) et de survie globale (SG). Chez les patientes âgées de 35 ans ou moins, il existait une répartition des sous-types moléculaires tumoraux différente de celle de la population plus âgée avec une proportion de sous-types luminaux B, triple négatif et HER2 plus importante (30 %, 19 % et 9 % contre 19 %, 11 % et 4 %) ainsi qu'un taux de tumeurs avec récepteurs hormonaux négatifs presque doublé (38 % contre 19 %).
