Cette étude a pour but d'évaluer l'impact sur la qualité de vie du traitement adjuvant du cancer du sein, et de définir quelle pourrait être la place du médecin généraliste dans le suivi des patientes. Nous avons adressé à deux cent quatorze patientes, issues d'un essai randomisé multicentrique, en rémission de leur maladie, des auto questionnaires standardisés portant sur l'évaluation de la qualité de vie (QLQ 30 - BR23), et la dépression (M.I.N.I). Elles ont toutes reçu, après chirurgie conservatrice du sein, soit une chimiothérapie et une radiothérapie concomitante (durée totale de trois mois), soit le même traitement administré de façon séquentielle (durée totale de six mois). Nous analysons également de façon comparative l'impact de la durée du traitement sur la survenue d'un épisode de dépression. Le suivi moyen après traitement était de sept ans et demi (4,3 à 9,7 ans). Sur les 138 patientes ayant répondu aux questionnaires (64,5%), plus de la moitié (53%) dit avoir traversé un épisode de dépression, toute modalité de traitement confondue. La durée du traitement était déterminante dans la survenue d'un épisode de dépression, puisque 63% des patientes ayant eu un traitement long, contre 43% un traitement court, en ont mentionné un. (p=0,043). La qualité de vie des patientes est globalement conservée, bien que certaines de ses dimensions comme la sexualité, la fatigue, la douleur et la peur d'une récidive nous interpellent. Nous discutons ces résultats qui montrent que la maîtrise des soins très techniques du cancer du sein devrait pouvoir être rapprochée du besoin d'humanité, d'accompagnement et d'écoute des patientes. Les efforts conjugués des différents acteurs de santé sont indispensables pour améliorer la qualité de vie des patientes et tenter de diminuer la dépression post-thérapeutique. Le médecin traitant a certainement un rôle fondamental dans la prise en charge du cancer et son suivi.
