La recomposition consiste à mettre à disposition les instruments chirurgicaux pour les chirurgiens ou soignants selon un listing prédéfini d'instruments. Elle représente une étape entièrement humaine, difficile, chronophage et sensible. Le facteur humain étant une source inévitable d'erreurs, cette étape peut donc engendrer des non-conformités avec un risque potentiel pour le patient. L'objectif de ce travail est de réaliser un état des lieux des outils de recomposition et de mettre en place un nouvel outil pédagogique. Les outils informatiques sous la forme des logiciels de traçabilité permettent un accès facilité aux listings des compositions et intègrent des documents multimédia (photos, vidéos..). Ces éléments sont d'une aide précieuse pour les agents de stérilisation. De plus, grâce à la traçabilité à l'instrument, chaque dispositif médical est suivi de manière individuelle au sein des compositions. La formation à la recomposition des agents de stérilisation est primordiale. Des outils pédagogiques sont disponibles sous plusieurs formes (plateau de jeu, escape game, cartes...). En intégrant le jeu, ils favorisent l'intérêt des participants. Au sein de notre établissement, un outil destiné à l'apprentissage de l'instrumentation chirurgicale a été créé sous la forme d'un quiz. Au total, 65 questions portant sur 11 catégories d'instruments ont été définies. Cet outil est apprécié de l'équipe puisqu'il permet un moment de partage sans notion d'évaluation. Suite aux remarques issues du questionnaire de satisfaction, une nouvelle catégorie d'instruments destinée à la chirurgie ophtalmologique a été créée. Afin d'utiliser les nouvelles technologies, une adaptation sous format connecté a été affichée dans l'unité. L'ensemble des outils informatiques, technologiques et pédagogiques permettent d'obtenir une qualité de recomposition conforme aux attentes des chirurgiens/soignants. Cette étape encore humaine doit donc faire l'objet d'ingéniosité de la part des équipes pharmaceutiques pour créer de nouveaux outils toujours plus ludiques et toujours plus modernes.

L'ensemble des normes et réglementations en matière de préparation des dispositifs médicaux stériles rappelle que la traçabilité doit faire partie intégrante du système du management de la qualité afin de garantir la conformité du processus de stérilisation et la sécurité des patients. L'objectif de cette étude est de décrire la mise en place d'une nouvelle méthode d'identification des plateaux d'instruments chirurgicaux par l'utilisation d'une plateforme de marquage laser. Notre service de stérilisation avait eu recours à plusieurs méthodes d'identification des plateaux d'instruments dont les supports d'identification n'étaient pas résistants au processus de stérilisation. Nous avons réalisé une enquête qui révèle que ces mêmes difficultés étaient rencontrées par d'autres services de stérilisation en France, en effet 48 % des établissements de santé participant à l'enquête n'étaient pas satisfaits des supports d'identification qu'ils utilisaient. L'acquisition d'une machine laser propre à l'établissement nous offre une capacité d'autonomie pour réaliser une identification sur place sans immobiliser le matériel tout en permettant d'obtenir une identification des plateaux d'instruments résistante au processus de stérilisation. Ce projet nous donne l'opportunité de réaliser en parallèle un audit complet de nos plateaux d'instruments et de renseigner de nouvelles informations sur notre logiciel de traçabilité. Notre étude évalue la durée de remise en conformité du parc opératoire en 9 mois et le retour sur investissement de notre projet sur 22 mois. Ce travail de remise en conformité du parc opératoire est une étape préalable indispensable pour envisager l'étude et la mise en place d'une traçabilité à l'instrument par marquage laser.

This book examines the nature and significance of the impact of population growth on the weIl-being of developing countries-in particular, the effects on economic growth, education, health, food supply, housing, poverty, and the environment. In addition, because family planning programmes often significantly affect population growth, the study examines the impacts of family planning on fertility and health, and the human rights implications of family planning programmes. In considering the book's conclusions about the impact of population growth on development, four caveats should be noted. First, the effects of population growth vary from place to place and over time. Thus, blanket statements about overall effects often cannot be made. Where possible, the authors note the contexts in which population effects are strongest and weakest. Second, all of the outcomes examined in this book are influenced by factors other than population growth. Moreover, the impact of population growth may itself vary according to the presence or absence of other factors. This again makes bl anket statements about the effects of population growth difficult. Throughout the chapters, the authors try to identify other relevant factors that influence the outcomes we discuss or that influence the impact of population growth on those outcomes.
