A l'heure où ils cherchent à renforcer leurs systèmes de santé, les pays se tournent de plus en plus vers les soins de santé primaires (SSP) pour trouver une orientation nouvelle et plus claire. Le Rapport sur la santé dans le monde 2008 analyse la façon dont les soins de santé primaires, fondés sur les principes d'accès universel, d'équité et de justice sociale, constituent une réponse essentielle aux défis sanitaires d'un monde en évolution rapide et aux attentes croissantes des pays et de leurs citoyens en matière de santé et de soins. Le Rapport identifie quatre séries indissociables de réformes des SSP qui visent à: assurer l'accès universel et la protection sociale afin d'améliorer l'équité en matière de santé; réorganiser la prestation des services en fonction des besoins et des attentes de la population; rendre les collectivités plus saines grâce à de meilleures politiques publiques; et remodeler le leadership pour la santé autour de gouvernements plus efficaces et de la participation active des principaux acteurs. Parallèlement, la nature des problèmes sanitaires change de manière spectaculaire. L'urbanisation, la mondialisation et d'autres facteurs accélèrent la transmission des maladies contagieuses à travers la planète et alourdissent la charge des maladies chroniques et non transmissibles. Le changement climatique et l'insécurité alimentaire auront des répercussions importantes sur la santé ces prochaines années, créant des obstacles énormes à la mise en oeuvre de mesures efficaces et équitables. Dans ces conditions, les services de santé ne peuvent plus continuer comme si de rien n'était. Nombre d'entre eux semblent dériver d'une priorité à court terme à l'autre, d'une manière de plus en plus fragmentée et sans savoir très bien où ils vont. Heureusement, le contexte international actuel est favorable à un renouveau des SSP. La santé mondiale fait l'objet d'une attention sans précédent. On observe un intérêt croissant pour l'unité d'action et de plus en plus de voix plaident en faveur de soins universels et complets, ainsi que pour que la santé soit présente dans toutes les politiques. Les attentes n'ont jamais été aussi fortes. En tirant parti de cette dynamique, investir dans les soins de santé primaires peut permettre de transformer les systèmes de santé et d'améliorer partout la santé des individus, de leurs familles et de la collectivité dans son ensemble. Pour tous ceux qui s'intéressent aux progrès que peut faire la santé au 21e siècle, la lecture du Rapport sur la santé dans le monde 2008 est donc incontournable.

This case study, focused on Indianapolis, examines a critical mass fund-raising and giving program. Community chests in many communities evolved into the present-day United Way.
