Nutrition related diseases such as some cancers, heart diseases and obesity belong to the most challenging health concerns of our time. Communicating intuitive and simple nutrition information by means of front-of-pack nutrition profile signposting labeling is increasingly seen as an essential tool in efforts to combat unhealthy food choices and improve public health. Consequently, much attention in policy and research is given to nutrient profiling methods and the determination of optimal nutrition criteria. Although consumer research on nutrition signpost labeling is now gradually appearing in the literature, the value and meaning of these labeling systems for consumers have received less attention. In the current debate a concise overview is lacking of the consumer perspective including relevant psychological phenomena in relation to much debated controversies surrounding these labels and their further development, such as the most effective type of signposting labeling system and varying stakeholder interests. Therefore, this paper aims to critically review the literature in the consumer domain of front-of-pack nutrition labeling in order to illustrate the strengths and weaknesses of this form of nutrition education from a consumer perspective.

During the past decade, tremendous growth has occurred in the use of nutrition symbols and rating systems designed to summarize key nutritional aspects and characteristics of food products. These symbols and the systems that underlie them have become known as front-of-package (FOP) nutrition rating systems and symbols, even though the symbols themselves can be found anywhere on the front of a food package or on a retail shelf tag. Though not regulated and inconsistent in format, content, and criteria, FOP systems and symbols have the potential to provide useful guidance to consumers as well as maximize effectiveness. As a result, Congress directed the Centers for Disease Control and Prevention (CDC) to undertake a study with the Institute of Medicine (IOM) to examine and provide recommendations regarding FOP nutrition rating systems and symbols. The study was completed in two phases. Phase I focused primarily on the nutrition criteria underlying FOP systems. Phase II builds on the results of Phase I while focusing on aspects related to consumer understanding and behavior related to the development of a standardized FOP system. Front-of-Package Nutrition Rating Systems and Symbols focuses on Phase II of the study. The report addresses the potential benefits of a single, standardized front-label food guidance system regulated by the Food and Drug Administration, assesses which icons are most effective with consumer audiences, and considers the systems/icons that best promote health and how to maximize their use.

Facts Up Front style nutrition labels are a front-of-package (FOP) nutritional labeling system that present key nutrient information on the front of packaged food and beverage products in an easy-to-read format. The authors conduct a large-scale empirical study to examine the effect of adoption of FOP labeling on the nutritional quality of products. The authors assemble a unique dataset on packaged food products (21,096 products, 9,083 brands, and 4,408 firms) in the U.S. across 44 categories over a period of sixteen years. They individually examine each new product introduced during the data time period to record FOP nutrition label information and use product-specific nutrient information to measure nutritional quality of products. By using a difference-in-differences estimator, the authors find that FOP adoption in a product category leads to an improvement in the nutritional quality of products in the category. They further find support for the moderating role of brand and product category characteristics. They argue that adoption of FOP in a product category serves as a source of “nutritional information clearinghouse” and empirically test for the role of salience of nutritional information as the mechanism that drives the improvement in the nutritional profile of the products. They find that subsequent to FOP adoption, food manufacturers improve the nutritional quality by lowering the content of calories and the nutrients to limit such as sugar, sodium, and saturated fat. Supplementary analyses are performed to rule out potential self-selection issues and a battery of robustness checks and falsification tests are conducted. The authors discuss the implications of the findings for public policy makers, consumers, manufacturers and food retailers.

Interpretive front-of-pack food labelling (FOPL) is a policy priority for promoting healthy diets. Research evidence indicates that consumers have a reasonable understanding of interpretive FOPL systems and their understanding improves with label familiarity and consistency within the market. A government-endorsed interpretive FOPL policy was found in 15 Member States of the WHO European Region and this report summarizes the evidence on their development and implementation to support policy-makers in navigating these processes. Most existing policies have been implemented under voluntary arrangements with variable penetration into the marketplace. Policy development that is led by government and based on formative research and that engages stakeholders and the public is most likely to lead to acceptable credible and effective policies. FOPL implementation is best supported by policy provisions that encourage widespread uptake of the system and allow for formal evaluation of both implementation and impact.
