This book describes common errors, including donor site selection, techniques to decrease scar contracture rate, and show improved long-term follow-up results. In addition, it will describe application of STSG on facial reconstructive surgery and aesthetic thin STSG for correction of irregular surface, color mismatch and old conspicuous engrafted margin. Full-thickness skin graft (FTSG) has been used for burn scar contracture of joint areas, including hand, foot, upper and lower extremities. It maximizes the range of motion of those areas and possesses a soft and elastic texture. However, compared to a conventional skin graft, more efforts are needed for graft take with a higher graft failure rate and limited donor site size, which makes surgeons hesitate to use FTSG. This book introduces techniques to decrease operation time, maturation period, pigmentation rate, and improve high graft take rate, skin texture and also newly introducing full thickness skin with subcutaneous fat composite graft technique and its concept. Recently, the development of various artificial dermis and cell culture techniques has greatly improved burn patients' survival rate. In this book the author’s experience with various types of artificial dermis, cultured epithelial cell therapy and its benefits for tendon and bone exposure limb salvage and variable cases of long-term follow-up results are shared with up-to-date techniques. The final chapter introduces unique aesthetic skin graft concepts to improve previous scar contracture lesion, dark pigmentation disorder, irregular surface and discoloration, conspicuous grafted margin and others.

This omission has now been rectified by Dr. Klasen. This book is an expansion of a M.D. thesis, written at the State Universi ty of Groningen, The Netherlands, and was guided by Professor A.J.C. Huffstadt, plastic surgeon, and Professor D. de Moulin, medical historian. Professor T. Gibson kindly revised the manuscript and wrote the epilogue. The author, Henk Klasen, is a remarkable man. As a general surgeon, he devotes all his interest and skills to traumatology and problems of phys iology and pathophysiology in surgery. With such talents it is natural that he also works parttime as one of the coordinators ofa modern burn unit. Among his hobbies are love of antiques and old books. This historical inclination has induced him to write the present book, in which he vividly describes the development of free skin grafting in its rel evant theoretical and practical aspects. During the last two decades, various surgical therapies have been used in the tre- ment of stable refractory vitiligo, but none of them became very popular because of their intrinsic drawbacks, and thus could not be adopted as a standard procedure. There has always been a need for a technique that gives best results without complications. The commonly used surgical procedures for the treatment of vitiligo have been narrated in short and their characteristic drawbacks have been pointed out. The c- lular graft technique is costly, requires a good laboratory and infrastructure, and except for being able to cover a wider vitiliginous area, the ultimate results are no better than tissue grafting technique. I used the small skin particles prepared from ultra thin or thin split thickness skin graft, the so called “microskin graft,” in the tre- ment of vitiligo effectively with good expansion ratio, i.e., small donor skin to treat big vitiliginous area. This procedure covers the bene? t of cellular and tissue graft, but without their inherent drawbacks. The microskin graft technique has been illustrated with minimum necessary text to explain the steps. This technique is very simple to learn and execute. It is extremely 2 cost effective and can treat very large area up to 1,500 cm in an operative session.
