L'évolution de la réglementation concernant le circuit du médicament vise à en améliorer la qualité, la sécurité et l'efficacité. Plusieurs dispositifs ont ainsi été créés afin d'inciter les établissements de santé à mettre en oeuvre une maitrise du risque de iatrogénie médicamenteuse, parmi ceux-ci, on note: la démarche de certification, le contrat de bon usage des médicaments et l'arrêté du 6 avril 2011 (RETEX). L'objectif de ce travail était de réaliser un état des lieux de toutes les mesures d'amélioration de la prise en charge médicamenteuse des patients hospitalisés au CHU de Rouen, en prenant comme exemple le service de Médecine Interne - Gériatrie - Thérapeutique (MIGT) de l'Hôpital de Saint-Julien. Les exigences réglementaires sont principalement respectées grâce à l'implication des équipes du service. Les exigences non respectées sont majoritairement dues au retrait de l' informatisation des prescriptions et à l'absence d'un préparateur en pharmacie dans le service alors que certaines exigences ne relèvent pas du service par leur spécificité. Le respect des exigences réglementaires concernant les services de soins semble ainsi liée au développement de 4 axes : la sensibilisation et l'implication des équipes dans le circuit du médicament, l'informatisation des prescriptions et des plans de soins, le déploiement de préparateurs en pharmacie et la présence régulière de pharmaciens cliniques dans les services de soins.

Le pharmacien est l'un des acteurs clés de lutte contre la iatrogénie médicamenteuse grâce au développement d'activités telles que la conciliation médicamenteuse (CM) et l'analyse pharmaceutique (AP). Les objectifs de notre étude étaient d'évaluer l'impact clinique et économique de ces activités sur un an au sein du service de Médecine interne gériatrie thérapeutique du CHU de Rouen. Au total, 196 patients ont bénéficié d'une CM à l'admission et 831 patients d'au moins une AP. 94 DNI (Divergences non intentionnelles) ont été détectées (soit 0,48 DNI par patient). Les DNI les plus rapportées étaient des omissions (72,3%) et des modifications de posologie (17,0%). Les principales classes thérapeutiques concernées étaient le système cardiovasculaire (SCV) (23,7%), les voies digestives et métabolisme (20,4%) et le système nerveux (SN) (12,9%). Entre 44% et 73% des DNI ont été évaluées comme susceptibles d'avoir un impact clinique sérieux à vital. 1779 prescriptions ont été analysées et 184 IP (Interventions pharmaceutiques) émises (soit k 0,22 IP par patient) avec un taux d'acceptation du prescripteur de 79,3%. Les IP concernaient essentiellement des problèmes de surdosage (29,9%) et d'interactions médicamenteuses (28,8%) amenant le pharmacien à émettre des propositions d'adaptation posologique (37,0%), d'arrêt de traitement (18,5%) et de suivi thérapeutique (17,9%). Les classes thérapeutiques concernées étaient principalement le sang et organes hématopoïétiques (23,8%), le SCV (20,2%) et le SN (18,6%). L'impact clinique potentiel des IP a été évalué comme très significatif voir vital à 23% et 43%. Nous avons estimé qu'une économie totale (coûts directs et indirects) de 29 897€ à 35 177€ était générée par ces activités. Les classes thérapeutiques les plus concernées par ces économies étaient les antibactériens (364€) et les substituts du sang et solutions de perfusions (360€). Le retour sur investissement de ces activités a ainsi été évalué entre 212 et 268%.
