Introduction : Les symptômes médicalement inexpliqués sont fréquents en médecine générale, et sont pourtant peu étudiés. Ils génèrent des coûts importants et posent des problèmes à la fois aux médecins (en termes de diagnostic, de thérapeutique et de ressenti) et aux patients (souvent insatisfaits). Ces symptômes ne font l'objet d'aucun enseignement spécifique. Nous avons étudié comment les internes en stage chez le praticien prenaient en charge ces patients sans diagnostic organique. Méthode : 15 entretiens semi-dirigés d'internes en stage chez le praticien. Résultats : Les internes interrogés prennent conscience au moment de leur stage de l'importance de cette problématique en médecine générale. Ils attribuent largement ces symptômes à une étiologie psychologique. Ils développent souvent de façon empirique des stratégies de prise en charge de ces patients, qui sont variables et très internes-dépendantes, incluant notamment des thérapeutiques médicamenteuses, le développement de capacités relationnelles et d'écoute, ou une approche biopsychosociale. Ils peuvent malgré tout être démunis face à ces symptômes, pour lesquels ils ne peuvent appliquer le modèle biomédical ayant dominé leurs études universitaires. Ils souhaiteraient par exemple une formation en psychologie médicale et sur la relation médecin-malade. Conclusion : Les symptômes médicalement inexpliqués permettent d'aborder plusieurs thèmes peu évoqués : la gestion de l'incertitude médicale, l'importance des déterminants psychologiques et des facteurs de personnalité du patient et du médecin. Les internes ont souvent amorcé une réflexion sur ces thèmes lors de leur stage chez le praticien. La richesse des données obtenues encourage à mettre en place une formation pluridisciplinaire sur cette question.

Les symptômes médicalement inexpliqués sont fréquents en médecine générale. Cette étude épidémiologique réalisée auprès de médecins généralistes enseignants au cours de leur consultation , a permis de déterminer les symptômes les plus fréquents et de mettre en évidence l’importance de la prise en charge globale des patients ayant des symptômes inexpliqués. Cette étude a en effet montré l’existence de facteurs favorisants, tel le sexe, la situation familiale, des évènements de vie traumatisants qui influenceraient l’apparition des symptômes inexpliqués mais aussi que le retentissement de ces symptômes inexpliqués sur la vie quotidienne était souvent sous- estimé.La mauvaise connaissance de ces symptômes entraine un diagnostic par exclusion et une prise en charge souvent inadaptée (prescriptions excessives d’actes médicaux et paramédicaux) plus par les étudiants que par les médecins. Il semble évident que l’enseignement des symptômes médicalement inexpliqués doit être intégré au programme des études médicales ainsi que celui de la prise en charge globale des patients.

Mental health and psychosocial support (MHPSS) is a term used to describe a wide range of actions that address social, psychological and psychiatric problems that are either pre-existing or emergency-induced. The Humanitarian Emergency Settings Perceived Needs Scale (HESPER) provides a quick, scientifically robust way of assessing the perceived serious needs of people affected by large-scale humanitarian emergencies, such as war, conflict or major natural disaster. Perceived needs are needs which are felt or expressed by people themselves and are problem areas with which they would like help. The HESPER Scale assesses a wide range of social, psychological and physical problem areas. However, it does not provide an answer as to whether, or how to, offer help. It simply identifies those serious problems that are common in a population. These problems should then be assessed and addressed in more detail. The HESPER Scale was developed by the World Health Organization and King's College London in order to fill several gaps in the humanitarian field.
