Afin de permette l'accès à ces innovations thérapeutiques et d'accompagner les professionnels dans cette nouvelle activité, nous avons réalisé une analyse des risques a priori ciblée sur la thérapie génique dans le cadre d'essais cliniques. La méthode d'analyse préliminaire des risques (APR) a été choisie afin d'identifier les risques en amont et de proposer un circuit sécurisé en adéquation avec les recommandations et la réglementation en vigueur. Cette analyse menée de manière globale, tant sur le circuit pharmaceutique que sur le parcours patient, nous a permis d'anticiper les différentes problématiques inhérentes à la manipulation d'OGM de classes I et II. La formation des professionnels de santé et l'accompagnement des patients dans ce domaine est essentielle et représente un vrai challenge pour cette activité qui en n'est qu'à ces débuts.

Les CAR-T cells appartiennent à une nouvelle classe de médicaments, les Médicaments de Thérapie Innovante (MTI), tels que définis par le règlement européen 1394/2007, et plus particulièrement à la catégorie des médicaments de thérapie génique. Les essais cliniques incluant ces CAR-T cells ne cessent de se multiplier à l'heure actuelle. Le CHU de Lille est sollicité pour participer à un essai clinique européen multicentrique de CAR-T cells nécessitant une production sur site. Confrontés à cette nouvelle demande, le but de ce travail est d'analyser les différentes solutions permettant une production de ces CAR-T cells au CHU de Lille conformément à la réglementation spécifique. Pour cela, nous avons, dans la première partie de ce travail, détaillé le contexte réglementaire encadrant les MTI et leur production, puis nous avons décrit la composition, l'efficacité, les effets secondaires et le processus de production des CAR-T cells et en particulier du protocole CARAMBA pour lequel nous sommes sollicités. La deuxième partie de ce travail consiste à établir les besoins actuels et futurs en matière de MTI au CHU de Lille. Nous avons ensuite dressés le bilan des différentes structures existantes sur site susceptibles d'accueillir la production de cet essai clinique. Ces structures ont ensuite été comparées à une check-list (centrée sur les locaux de production) élaborée à partir des « guidelines on Good Manufacturing Practice » spécifiques à la production des MTI afin d'établir leurs points de conformité. Enfin, nous discuterons des solutions disponibles quant à la prise en charge de la production de cet essai clinique au CHU de Lille.

En France, ces produits obéissent au règlement européen n°1394/2007 sur les médicaments de thérapie innovante, dont la mise en place a marqué une grande avancée pour promouvoir le développement de ce type de produits et faciliter l'accès au marché européen. Huit ans après la mise en place de ce règlement, les mesures prises ont réussi à montrer leur efficacité : les essais cliniques sont encourageants et quatre médicaments de thérapie génique sont parvenus à obtenir une autorisation de mise sur le marché. Cependant, les laboratoires pharmaceutiques font face à de nombreuses difficultés de développement et à un système réglementaire trop complexe et pas toujours adapté pour évaluer ce nouveau type d' « OGM-médicaments ». La réglementation nécessite donc d'être ajustée, en établissant un cadre réglementaire européen harmonisé entre les Etats membres et en cohérence avec les autorités et organismes payeurs. Ces ajustements seront nécessaires pour atteindre leur objectif principal de mettre à disposition des médicaments de thérapie génique sûrs et efficaces pour les patients.
