Au cours de sa 3ème phase, l'enquête santé mentale en population générale : images et réalités a été réalisée à Brest du 17 au 29 mars 2008. Il s'agit d'une enquête nationale réalisée sur près de 50 sites différents soit 54 000 personnes interrogées. L'enquête comporte deux objectifs : décrire les représentations de la « folie », de la « maladie mentale » et de la « dépression » ainsi que des modes d'aide et de soins (=IMAGES) et évaluer la prévalence des principaux troubles mentaux dans la population interrogée (= REALITE). Sur 900 sujets interrogés, 416 personnes ont été diagnostiquées pour au moins une des pathologies recherchées. Cela correspond à 46.2% de l'échantillon. Les représentations sociales de la maladie mentale reposent encore largement sur les notions de « folie », dangerosité, d'incurabilité et d'exclusion. Cette image pénalise les personnes souffrant de troubles psychiques. Le rôle du médecin généraliste trouve alors ici toute sa place dans le dépistage, le diagnostique, le traitement et l'orientation du patient psychiatrique, dans son rôle d'acteur central du soin apporté aux patients. Cette prise en charge du patient doit être globale avec un soutien et une collaboration de la psychiatrie du secteur.

Contexte : Nous savons que l’état de santé des patients hospitalisés en psychiatrie est moins bon que celui de la population générale (6,8). Or, il est évident qu’un patient hospitalisé quel que soit sa pathologie a par définition un moins bon état de santé qu’une personne de la population générale sans comorbidités. Il est donc logique de confronter les patients hospitalisés en psychiatrie avec la population générale suivie pour des comorbidités, c’est-à-dire en médecine générale.Méthode : Etude prospective et descriptive sur l’ensemble des patients hospitalisés à l’Unité de Santé Mentale (USM) du Centre Hospitalier de Calais de mai à octobre 2012 inclus. Chez chaque patient hospitalisé a été effectué une ou plusieurs consultations de médecine générale au cours duquel sont recueillis des résultats de consultations. Elles sont comparées aux données recueillies en 2009 par le réseau de médecins informatisés du Nord-Pas-de-Calais utilisant le Dictionnaire des résultats de consultation (DRC) de la Société française de médecine générale (SFMG).Résultats : En excluant les résultats déjà identifiés chez les patients atteints de pathologies psychiatriques (tabagisme, alcoolisme, toxicomanie, tentative de suicide), d’autres résultats sont augmentés par rapport à la médecine générale. Certains s’expliquent principalement par un effet indésirable des médicaments (constipation (12,7% IC95% [9,3 - 17])), d’autres par la somatisation de la pathologie psychiatrique (eczéma (4,1%, IC95% [2,3-7,1])), ou par un dépistage systématique lors de l’hospitalisation en psychiatrie (vaccination (26,3% IC95% [21,6 - 31,6]), hypothyroïdie (6,3%, IC95% [4-9,8]), anomalie biologique sanguine (5,7%, IC95% [3,5 - 9]), anémie non ferriprive (5,1%, IC [3 - 8,3])) ou l’organisation du service (plaie (4,4%, IC [2,5-7,5])).

How the insane asylum became a laboratory of democracy is revealed in this provocative look at the treatment of the mentally ill in nineteenth-century France. Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.
