Spanish Abstract: Desde el amor hasta en la guerra, ya estamos entrando a la sociedad de robots (Krings & Weinberger, 2018) de modo que los robots (robots asistentes, robots sociales, robots conversacionales, robots de compañía, robots al lado de la cama, robots terapéuticos, robots cirujanos, robots ingeribles, nano-robots, robots para el cuidado de ancianos) y la inteligencia artificial jugarán --ambas, en combinación-- un rol central en todos los aspectos de nuestra siempre cambiante sociedad contemporánea. La robótica aplicada a la salud en general y la robótica médica en particular están abriendo nuevas oportunidades para mejorar el diseño y los procesos tanto en el ámbito del hospital como de los hogares. Estos fenómenos tendrán un profundo impacto en el rol y en las responsabilidades de la profesión médica y en la profesión de enfermería, las cuales tendrán que trabajar en conjunción con la tecnología robótica. ¿Qué habilidades serán imperativas para que los estudiantes de medicina y de enfermería puedan realizar sus tareas en el futuro próximo? ¿Qué márgenes de autonomía se les asignará a los robots en asistencia en el cuidado de ancianos, en rehabilitación, en la atención de la demencia o en la atención y cuidados de niños con trastorno del espectro autista (TEA), por ejemplo? ¿Cómo vamos a definir el término “asistencia” en la era de robot? Además, los avances en la computación aplicada a la visión y al procesamiento del lenguaje oral están haciendo posible ofrecer robots socialmente inteligentes diseñados de acuerdo a las nuevas necesidades del mercado de la atención de la salud. ¿Estamos ya preparados para trabajar con robots que piensan y que aprenden a pensar por sí mismos? ¿Estamos dispuestos a aceptar ser parte de equipos de trabajo humanos-robots? En este aspecto, la Robo-Ética es un campo emergente de indagación en busca de respuestas a cuestiones que ya están aflorando en los sistemas de salud del mundo (Seibt, 2017; Leventi, et al, 2017; Wohl, 2014). This book disentangles the issues in connection with the advancement of Health Technology Assessment (HTA) and its interface with health policy. It highlights the factors that should shape its progress in the near future. Interdisciplinary and critical views from a number of professionals are put together in a prescient order to cast some light and make recommendations as to the next steps HTA should take to be fit for purpose. A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.
