CONTEXTE : Parmi les plantes médicinales, le cannabis est probablement l'une des plus anciennes. Aujourd'hui l'usage médical du cannabis est autorisé dans 21 pays sur les 27 que comptent l'Union Européenne. En mars 2021 la France a débuté une expérimentation. Elle a deux objectifs: tout d'abord d'évaluer en situation réelle, le bon déroulement du circuit de prescription et de délivrance entre les professionnels de santé, médecins et pharmacies, et les patients ; deuxièmement recueillir les premières données françaises sur l'efficacité et la sécurité de l'utilisation du cannabis dans un cadre médical. Ainsi, l'évaluation de cette expérimentation permettra de décider d'une possible généralisation du cannabis médical en France. OBJECTIF : L'objectif de l'étude est de réaliser un état des lieux des représentations du cannabis médical chez les médecins généralistes. Ce travail cherche à comprendre leurs attentes et mieux cerner le rôle qu'ils souhaitent avoir. L'étude s'intéresse également aux préjugés du cannabis et les réticences face à son usage. MÉTHODE : Il s'agit d'une étude qualitative réalisée par entretiens semi-dirigés auprès d'un panel diversifié de médecins généralistes installés en Auvergne. Inclusion jusqu'à saturation théorique des données. Analyse thématique. RÉSULTATS : L'étude a permis d'explorer les connaissances sur le cannabis médical, et de mettre en évidence une confusion globale autour de cette substance. En effet, dans ses représentations, un amalgame est fait d'une part entre le cannabis dit thérapeutique et le cannabis médical, mais d'autre part entre les produits à base de CBD en vente libre et le cannabis médicament. Le besoin le plus récurrent exprimé par les médecins généralistes, est celui de la prise en charge antalgique. Leur intérêt est centré sur l'obtention d'une nouvelle thérapeutique pour leur patient. De plus, ils sont en attente de données validées et de connaître les modalités de prescription, d'usage et de surveillance. Il ressort également que les médecins ont très peu de craintes vis à vis du cannabis médical et sont enclins à sa généralisation. Leurs appréhensions portent sur la confusion par les patients entre les différents types de cannabis, ainsi que les possibles mésusages et addictions. Enfin, ils souhaitent pouvoir suivre une formation avant d'intégrer le cannabis médical dans leur pratique courante. CONCLUSION : Cette étude aura mis en évidence l'importance d'utiliser une terminologie précise entre les différents intervenants, afin d'éviter toute confusion. Les médecins généralistes ont exprimé peu de crainte vis à vis du cannabis à usage médical. Leurs attentes portent essentiellement sur les propriétés antalgiques de cette plante, afin de proposer une alternative à leurs patients en échec thérapeutique. Le cannabis médical est la source de beaucoup de fantasmes chez ses défenseurs et ses détracteurs, mais il est certain que cette plante ait beaucoup à offrir à la médecine de demain.

En France, le cannabis est la substance psychoactive illicite la plus consommée et les problématiques associées à cette consommation ne peuvent être ignorées. Principal acteur des soins de premier recours, le médecin généraliste est le mieux placé pour dépister cet usage. Le dialogue entre un patient et son médecin traitant sur la question du cannabis n'est pourtant pas aisé. L'objectif de cette étude est d'explorer les freins et les motivations des patients pour aborder la question du cannabis avec leur médecin et d'étudier ce qu'ils attendent de cette interaction. Nous avons réalisé une étude qualitative par entretiens semi-structurés auprès de 15 patients consommateurs réguliers de cannabis en région Auvergne Rhône Alpes. Une analyse thématique a été effectuée à partir des données retranscrites. La relation de confiance avec leur médecin ou le sentiment de prendre un risque à consommer du cannabis étaient les deux motivations évoquées par les patients pour aborder la question du cannabis. Ils identifiaient de nombreux obstacles au dialogue. Certains étaient inhérents au patient comme la peur d'être jugé, la peur du non-respect du secret professionnel ou l'absence de problématique ressentie. D'autres étaient liés au médecin comme une attitude parfois inadaptée, des capacités limitées de prise en charge ou une méconnaissance des problématiques liées au cannabis. Les attentes des consommateurs de cannabis en matière de prise en charge concernaient à la fois l'attitude du médecin, fondée sur l'écoute et l'objectivité, et son action pendant la consultation. Ils imaginaient que le médecin devait repérer l'usage et donc poser la question pour les accompagner dans leur consommation. Parfois ils pensaient au sevrage et attendaient que le médecin les aide ou les oriente vers une structure adaptée. Malgré de nombreux obstacles, les patients sont prêts à discuter de leur consommation de cannabis avec leur médecin. Pour cela, le médecin ne doit pas hésiter à poser la question. Il semble nécessaire de développer la formation des médecins généralistes sur cette problématique pour qu'ils se sentent moins démunis et qu'ils osent ouvrir le dialogue
