Les traitements orthodontiques avec extraction d’une incisive mandibulaire ont été décrits depuis longtemps mais sont encore peu utilisés par les praticiens. Les indications sont limitées à des situations bien précises, notamment la classe I avec encombrement modéré et dysharmonie dento-dentaire par excès mandibulaire. Ils peuvent également être proposés dans le traitement de compromis des classes III légères à modérées dans le but d’améliorer le rapport incisif. - L’analyse rigoureuse de la situation clinique (facteurs dentaires, parodontaux, esthétiques, etc.) orientera le praticien dans le choix de l’incisive à extraire. Ce choix est important puisqu’il conditionne en partie le résultat final. - Les répercussions occlusales de l’extraction de l’incisive devront être prévisualisées et validées par un set-up. - La présence de triangles noirs en fin de traitement est décrite comme l’inconvénient majeur de cette thérapeutique. Cependant, une préparation orthodontique du site d’extraction peut s’avérer efficace dans la réduction de ces embrasures gingivales inesthétiques
