It is the position of the National Association of School Nurses (NASN) that school districts develop written medication administration policies and procedures that focus on safe and efficient medication administration at school by a registered professional school nurse (hereinafter referred to as school nurse). Policies should include prescription and non-prescription medications, and address alternative, emergency, research medication, controlled substances, and medication doses that exceed manufacturer's guidelines. These policies shall be consistent with federal and state laws, nursing practice standards and established safe practices in accordance with evidence based information. The "Individuals with Disabilities Education Act, and Section 504," mandate schools receiving federal funding to provide "required related service," including medication administration (O'Dell, O'Hara, Kiel, & McCullough, 2007). There is a myriad of situations and contributing factors that can lead to medication errors in a clinical setting, and the school health office is no exception. Weather it is an over the counter medication or prescription the 5 Rights of medication administration must be observed in all health care settings (Randolph, 2013). Medication errors can lead to catastrophic outcomes for all stakeholders. This study examined a concern regarding the lack of a mandatory patient identification protocol in the school health system and how the nurse identifies each child prior to medication administration. Thus, a proposal for medication administration change in the school health system is critically needed. To implement change, 44 school nurses in the Paradise Valley Unified School District (PVUSD) individually conducted a one year retrospective pre-implementation study of all medication errors in their schools. The results will be compared to post-implementation as well as ongoing management for other risk factors. After data was validated a trial implementation plan was conducted lasting one school year quarter. Nurse's progress questionnaire was sent via electronic communication to all participants regarding progress and suggestions for any plan modifications. There is an ongoing gap analysis conducted by the task leaders to assist in the seamless transition of policy change. The fully developed plan will be implemented in the 2015-16 school year with ongoing management for risk factors and quality control. The relevance to safety in nursing clinical practice includes adoption of the new medication administration protocol in school heath office settings in all schools nation-wide to promote and ensure patient safety and integrity. If adequate confirmation of the child by using school photo identification card (ID) is matched to the medication prior to medication administration errors in the school health office will be eliminated.
