"Il appartient donc au médecin obstétricien référent, ou à l'équipe obstétricale référente, qui connaît bien la patiente, d'évaluer, au cas par cas, en fonction de la situation et de son expérience personnelle, la possibilité du suivi à domicile par la sage-femme libérale". Ce type de suivi suppose des compétences professionnelles et engage des responsabilités, morale d'abord, ainsi que celles relevant d'une observation sans faille de la loi, du code de déontologie et des règlements, comme ceux de la Sécurité Sociale que l'on pourra trouver dans cette étude en annexes. Comment se déroule le suivi à domicile ? Des objectifs à la naissance de l'enfant, en passant par l'organisation de la première visite et des suivantes dans leurs différents volets, sans oublier la circulation de l'information ascendante, remontant au médecin obstétricien, ni celle descendante, que la sage-femme donne à la patiente dans un but double d'information et de prévention, les maîtres mots sont, entre autres : sécurité, sécurisation, prévention, information, travail en équipe et, au delà du contrat, économie publique. Les enquêtes menées auprès des patientes, des sages-femmes ainsi que des médecins montrent clairement la satisfaction commune, pour des motifs différents, à l'endroit du suivi à domicile des grossesses à risques. L'aspect de la courbe illustrant la population des sages-femmes libérales en France pourrait inquiéter. En effet, durant les dix années 1977-1987, les effectifs sont passés de 2500 à 1250 (chiffres arrondis). La dernière décennie voit une stabilisation se dessiner autour de 1250. Y a-til désaffection par manque d'information, de courage ç s'engager de la part de jeunes diplômés ? L'effectif des sages-femmes libérales est appelé, dans la conjoncture des années de fin de siècle, à une certaine renaissance dont l'auteur ne doute pas puisque 1695 sages-femmes libérales sont en activité en France au 1er janvier 1996 - chiffre à rapprocher des 1250 de l'année 1987. La dernière partie de l'étude montre les intérêts, dans des domaines variés, du suivi à domicile. Le travail de l'équipe, dans laquelle les obstétriciens référents pratiquant en maternité et les sages-femmes libérales composent un microcosme oeuvrant dans le sens d'une sécurité accrue de la santé de la mère et de l'enfant, loin d'occasionner des frais supplémentaires à la société, par les hospitalisations coûteuses qu'elle permet d'éviter lorsqu'elles ne sont pas impératives, contribue à réaliser à moindres frais un service judicieusement organisé.

The Violence of Modernity turns to Charles Baudelaire, one of the most canonical figures of literary modernism, in order to reclaim an aesthetic legacy for ethical inquiry and historical critique. Works of modern literature are commonly theorized as symptomatic responses to the trauma of history. In a climate that tends to privilege crisis over critique, Debarati Sanyal argues that it is urgent to rethink literary experience in terms that recall its contestatory potential. Examining Baudelaire's poems afresh, she shifts the focus of critical attention toward an account of modernism as an active engagement with violence, specifically the violence of history in nineteenth-century France. Sanyal analyzes a literary current that uses the traditional hallmarks of modernism—irony, intertextuality, self-reflexivity, and formalism—to challenge the historical violence of modernity. Baudelaire and the committed ironists writing in his wake teach us how to read and resist the violence of history, and thereby to challenge the melancholy tenor of our contemporary "wound culture." In a series of provocative readings, Sanyal presents Baudelaire's poetry as an aesthetic form that contests historical violence through rhetorical strategies of complicity, counterviolence, and critique. The book develops a new account of Baudelaire's significance as a modernist by dislodging him both from his traditional status as a practitioner of "art for art's sake" and from his more recent incarnation as the poet of trauma.
