Contexte : Alors que 85% des Français souhaiteraient finir leur vie au domicile, seuls 26% y parviennent réellement. Les politiques de santé tentent d'orienter leurs actions en faveur d'une prise en charge de la fin de vie au domicile. De nombreux obstacles persistent. L'objectif de cette étude était d'obtenir des pistes d'amélioration de la prise en charge de la fin de vie à domicile sur le territoire du Douaisis. Méthode : Étude qualitative utilisant la technique du groupe nominal à la recherche d'un consensus d'experts. La question nominale était « Comment optimiser la prise en charge de la fin de vie à domicile sur le territoire du Douaisis ? ». Résultat : Ce groupe était constitué de 7 experts de la fin de vie sur le Douaisis. Trente propositions ont été émises. Il en restait 13 après la phase de clarification. Les 5 propositions consensuelles jugées prioritaires après vote étaient : élaboration d'un dossier unique, mise en place de formations et informations sur l'interdisciplinarité, la coordination et les soins palliatifs, évaluation des besoins psychologiques du patient et de son entourage, identification d'un référent médical et convenir d'un moyen de communication médecin/IDEL. Conclusion : La mise en place de ces propositions permettrait de délivrer des soins coordonnés de qualité dans le but de respecter davantage le souhait des patients. Une autre étude serait nécessaire pour leurs mises en oeuvre pratiques. La finalité étant de tendre vers une prise en charge de la fin de vie idéale.

Contexte : Enjeu majeur de santé publique, la fin de vie fait partie des situations complexes. La politique de santé actuelle prévoit de développer les prises en charge de la fin de vie à domicile. Dans le cadre du projet de plateforme territoriale d'appui, une thèse précédente a abouti à cinq propositions d'optimisation de la fin de vie à domicile sur le Douaisis : dossier unique, formations pluriprofessionnelles, évaluation des besoins psychologiques, identification du référent médical, moyen de communication entre infirmiers et médecins généralistes. L'objectif de cette étude était d'établir des actions concrètes pour la mise en place de ces cinq axes d'amélioration. Méthode : Recherche d'un consensus d'experts par méthode Delphi adaptée à 2 rondes de questionnaires entre décembre 2019 et février 2020. Le groupe d'experts était constitué d'acteurs du parcours de santé du Douaisis ayant déjà vécu une situation de fin de vie à domicile. Les réponses ont été récoltées sur la plateforme LimeSurvey©. Les consensus ont été déterminés par des échelles de Likert ou par fréquence des votes. Résultats : Soixante-cinq experts ont été sollicités, 23 ont répondu au 1er questionnaire et 13 ont répondu au 2nd questionnaire. La hiérarchisation des actions a permis l'élaboration de fiches de recommandations pour les cinq propositions d'optimisation. Conclusion : Les recommandations émises ont vocation à être mises en oeuvre dès 2020 afin d'engager un changement des pratiques et de respecter davantage la volonté des personnes en fin de vie.

This report provides the most comprehensive and rigorous analysis of Iraqi income and expenditure in several decades. The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
