Objectif : Évaluer l'efficacité et la sécurité de la tentative de voie basse après césarienne (TVBAC) en cas de déclenchement par oxytocine, notamment en fonction du score de Bishop. Matériel et méthodes : Cette étude observationnelle rétrospective a eu lieu du 1er janvier 2013 au 31 mars 2017 au Centre Hospitalo-Universitaire Grenoble Alpes. Toutes les femmes avec utérus unicicatriciel présentant une grossesse mono-fœtale à terme et sans contre-indication à la TVBAC ont été incluses. En cas de travail spontané, la patiente était exclue. La décision entre déclenchement par oxytocine et césarienne était prise par l'obstétricien de garde et par la patiente après information loyale et appropriée, en fonction des antécédents et des constatations cliniques à l'entrée. Résultats : Sur les 966 patientes avec accord de voie basse, 248 ont été déclenchées avec un taux d'accouchement voie basse (AVB) de 58,5%. Il était de 81,7% chez les patientes avec col favorable et de 47% chez celles avec col défavorable. Huit cas de rupture utérine ont été rapportés en cas de déclenchement, soit 3,2%, ce qui constituait la principale différence quant à la morbidité maternelle avec le groupe des césariennes sans urgence (p = 0,049). La morbidité néonatale était faible dans les deux groupes. Discussion : Notre taux d'AVB en cas de déclenchement par Syntocinon® est satisfaisant. Néanmoins, la morbidité maternelle induite en matière de rupture utérine n'est pas négligeable. En conséquence, l'information au préalable de la patiente sur le taux de succès de la TVBAC, à moduler avec les facteurs de risque connus, et sur le risque de rupture utérine, est primordiale.
