Le diabète est une affection très courante, et sa prévalence accuse une hausse très sensible en France, près de 50% de l’ensemble de la population des diabétiques ont plus de 65 ans. Cette maladie métabolique chez le sujet âgé constitue une pathologie spécifique où les conséquences de l’hyperglycémie chronique s’additionnent aux co-morbidités liées au vieillissement constituant ainsi un problème majeur de santé publique. L’évaluation gérontologique est primordiale pour une meilleure prise en charge du sujet diabétique. Nous avons réalisé une étude descriptive sur 100 patients diabétiques hospitalisés dans les services de gériatrie du pôle de gérontologie du CHU de Nice. Les résultats montrent que ces patients présentent un profil très hétérogène dominé par la forte présence des complications cardiovasculaires qui conditionnent le pronostic vital (66% des patients), l’association de l’hypertension artérielle et le diabète chez le sujet âgé est très fréquente, elle intéresse 68% des patients. La dénutrition touche 77% des sujets, cette dernière constitue un facteur majeur de morbi-mortalité. Par ailleurs l’insuffisance rénale est présente chez plus de 44% des patients et les troubles cognitifs sont retrouvés chez plus de 56% d’entre eux. Le médecin généraliste est le coordinateur essentiel dans le suivi médical de ces patients. Par ailleurs notre étude confirme un pourcentage très élevé de patients mal équilibrés dont l’hémoglobine glycosylée dépasse les 8% (59% des patients), de même l’hémoglobine glycosylée dans la population étudiée est en moyenne de 7.76% reflétant un équilibre glycémique médiocre. L’indication de l’insuline est retenue chez 51.10% des patients (insuline exclusive dans 45.7%).Le principal obstacle à l’utilisation des anti-diabétiques oraux est la prévalence importante de l’insuffisance rénale chez les sujets âgés, l’évaluation de cette dernière est une nécessité impérative. Les objectifs glycémiques et les choix thérapeutiques sont fonction de plusieurs critères et ces objectifs sont déterminés au cas par cas, ou la prise en charge et la prévention des complications métaboliques restent au premier plan tout en gardant à l’esprit la notion de qualité de vie. La santé n’a pas de prix mais elle a un coût, la prise en charge du sujet âgé diabétique et la maîtrise des dépenses de santé sont des priorités nécessaires d’une bonne politique de santé publique.

Since then, it has constantly been republished and a glossary has been added in 1998. This version is the first complete edition of the book in English. The main idea of this new interview technique is to help a teacher, a supervisor or a researcher to guide the student, the practitioner or the subject to give a fine retrospective description of an action. As it is only the subject that has access to this mental or physical action, the goal is to help the subject with the verbalization, putting the implicit knowledge of this action in words. Yet, to achieve this verbalization, we need guidance, because we don't know how to give such a description ourselves. If we leave the interviewee without guidance, he risks remaining in the implicit, speaking in a general way, or in an incomplete or imprecise way, commenting instead of describing. The explicit and precise description we aim is the opposite of the implicit, meaning a fine, detailed and complete way of putting the action in words. The final aim is to elucidate what the subject did to seek his goal
