Introduction : la longévité humaine a nettement augmenté, entraînant un vieillissement de la population et donc une augmentation du nombre de personnes âgées voire très âgées. En 2016 en France, il existait 21 000 centenaires ; et les projections nous prédisent 270 000 centenaires en 2070. La prise en charge médicale du patient centenaire s'inscrit dans une approche positive et globale, nécessitant une attention particulière. Objectifs : ce travail a eu pour objectif principal de repérer des critères de fragilité chez les patients centenaires vivant à domicile et en Établissement d'Hébergement pour Personnes Âgées Dépendantes (EHPAD) pris en charge par des médecins généralistes de Marseille et des environs en 2020. Les objectifs secondaires étaient de décrire puis de comparer leur profil gériatrique et leur prise en charge médicale et thérapeutique selon leur lieu de vie, l'existence d'une poly médication, d'une dénutrition ainsi que de leurs comorbidités. Méthode : réalisation d'une étude observationnelle, multicentrique, descriptive à partir des données démographiques, sociales, cliniques, gériatriques, biologiques et radiologiques transmises par les médecins coordonnateurs des EHPAD et les médecins traitants des patients à domicile. Résultats : au moins une fragilité ou un syndrome gériatrique était présent chez les 22 centenaires inclus, à savoir la perte d'autonomie, l'existence de troubles cognitifs, d'un syndrome dépressif, de chutes à répétition ou troubles de la mobilité, d'une dénutrition, d'une poly médication, d'une poly pathologie ou d'un isolement social. L'âge moyen était de 101 ans, il y avait 77.3% de femmes, 59.1% des patients vivaient en EHPAD. Tous les patients étaient dépendants sur les activités de la vie quotidienne. Chaque patient présentait en moyenne 5 comorbidités et consommait en moyenne 6.5 médicaments, amenant à 86.4% la proportion de patients rentrant dans les critères de poly médication. The Governor's Dilemma develops a general theory of indirect governance based on the tradeoff between governor control and intermediary competence; the empirical chapters apply that theory to a diverse range of cases encompassing both international relations and comparative politics. The theoretical framework paper starts from the observation that virtually all governance is indirect, carried out through intermediaries. But governors in indirect governance relationships face a dilemma: competent intermediaries gain power from the competencies they contribute, making them difficult to control, while efforts to control intermediary behavor limit important intermediary competencies, including expertise, credibility, and legitimacy. Thus, governors can obtain either high intermediary competence or strong control, but not both. This competence-control tradeoff is a common condition of indirect governance, whether governors are domestic or international, public or private, democratic or authoritarian; and whether governance addresses economic, security, or social issues. The empirical chapters analyze the operation and implications of the governor's dilemma in cases involving the governance of violence (e.g., secret police, support for foreign rebel groups, private security companies), the governance of markets (e.g., the Euro crisis, capital markets, EU regulation, the G20), and cross-cutting governance issues (colonial empires, "Trump's Dilemma"). Competence-control theory helps explain many features of governance that other theories cannot: why indirect governance is not limited to principal-agent delegation, but takes multiple forms; why governors create seemingly counter-productive intermediary relationships; and why indirect governance is frequently unstable over time.
