Introduction : les maladies rares, malgré leurs faibles prévalences individuelles, touchent au total un grand nombre de malades et concernent particulièrement la population pédiatrique. Leur prise en charge s'est considérablement améliorée ces dernières années avec l'arrivée de nouveaux traitements, modifiant leur évolution sans être totalement curatifs. Comme cela soulève de nombreuses questions et dilemmes d'ordre éthique, nous avons réalisé une étude sur le respect des principes fondamentaux de l'éthique médicale dans la prise en charge des maladies rares en pédiatrie. Matériel et méthodes : nous avons réalisé une étude qualitative dans un service de Neurologie pédiatrique français. Nous avons inclus des soignants et des parents d'enfants suivis pour maladies rares et bénéficiant d'une thérapeutique innovante. Nous avons réalisé des entretiens semi dirigés et, après transcription, nous avons procédé à une analyse informatisée et manuelle. Résultats : 26 participants ont été inclus. Six grands thèmes ont été abordés : les maladies rares, la recherche scientifique et médicale, la prise en charge générale, les traitements spécifiques innovants, le dépistage néonatal et le coût de ces traitements. Dans les discours, l'enfant occupait une place centrale. Une importance particulière était donnée aux notions de connaissance, à l'information et à la relation médecin/famille. Une place importante était accordée aux objectifs des traitements avec en premier lieu l'amélioration de la qualité de vie. Il apparaissait également une notion de satisfaction concernant la prise en charge globale actuelle de ces maladies. Conclusion : notre étude suggère que nos prises en charge actuelles, incluant les thérapeutiques innovantes, respectent les quatre principes éthiques fondamentaux et ce, à la fois dans le discours des soignants et dans celui des parents.
