Les moyens thérapeutiques ayant prouvé leur efficacité sur le pronostic de l'infarctus du myocarde (IDM) des sujets jeunes sont encore en évaluation chez les patients âgés. Le but de cette étude est de décrire les caractéristiques de l'IDM chez les plus de 80 ans et d'évaluer leur prise en charge en Cardiologie à l'hôpital de Montreuil. Un recueil rétrospectif de tous les IDM en phase aiguë a été réalisé concernant les sujets âgés de 80 ans ou plus entre 1998 et 2004. Les caractéristiques des patients, leur prise en charge, leur évolution hospitalière et leurs traitements de sortie ont été recensés puis comparés selon leur délai d'admission inférieur ou supérieur à 12 heures, et selon la stratégie thérapeutique : interventionnelle ou non. Résultats : 169 patients ont été inclus (61 % de femmes). L'hypertension artérielle constitue le premier facteur de risque (56%). 60% des patients « moins de 12 h » sont arrivés par le SAMU (p=0,0001). 71 % présentaient une douleur thoracique typique. Dix patients (5,9%) ont été thrombolysés par le SAMU. 60% des patients ont eu une coronarographie et 74,5% d'entre eux, une angioplastie avec pose de stent. La mortalité hospitalière globale a été de 18% : 14% pour les « moins de 12 h » et 25% pour les « plus de 12 h » (p=0,03). Les patients « plus de 12 heures » ont plus souvent évolué vers l'insuffisance cardiaque que les « moins de 12 heures» (p = 0,009), et ont présenté plus de fibrillation auriculaire (p=0,03). La mortalité a été de 14,5% dans le groupe « traitement interventionnel », 21 % dans le groupe « traitement médical » (p=0,08). En conclusion, l'admission précoce des patients octogénaires est l'enjeu principal pour réduire leur mortalité et les complications ultérieures; la reperfusion coronaire par angioplastie primaire est réalisable au delà de 80 ans, et est associée dans cette étude à une réduction non significative de la mortalité
