Introduction : la prise en charge de la douleur est un enjeu capital en médecine mais la consommation d'antalgiques, malgré les particularités de prescriptions liées à l'âge et aux comorbidités, reste encore peu étudiée chez la personne âgée. Il a paru utile de réaliser une enquête afin d'évaluer les pratiques de prescriptions d'antalgiques chez ces patients. Méthode : en 2018, la SFGG a lancé une étude descriptive transversale multicentrique. Un questionnaire était transmis aux gériatres dans différentes structures gériatriques. Tout patient ayant un traitement antalgique était éligible. Les données cliniques étaient recueillies sur le dossier patient. Résultats : 830 patients purent être inclus, répartis de façon relativement équivalente entre les différents types de structures. 69% présentaient des troubles de la communication verbale; 53% souffraient de douleurs chroniques. 91,6% des patients prenaient du paracétamol ; 10 % du tramadol. 13% prenaient de la morphine et 7% du fentanyl en patch. 27% des patients avaient une prise en charge non médicamenteuse. Discussion : les posologies étaient presque systématiquement adaptées à la gériatrie. On notait des tendances de prescription en fonction du type de structure concernant les opioïdes, par exemple pour le fentanyl, prescrit à 33 patients en EHPAD, contre un seul patient en court-séjour, ou pour la morphine, prescrite à 3 fois plus de patient en CSG qu'en EHPAD. 10% n'étaient pas évalués sur le plan de la douleur par une échelle et plus de la moitié l'étaient à l'aide d'une échelle non adaptée aux personnes âgées présentant des troubles de communications. Conclusion : cette étude, bien que comportant des limites, permet de mettre en avant des tendances de prises en charge différentes selon les types de structures pouvant amener à des hypothèses notamment sur les contraintes organisationnelles inhérentes à chacune, et qu'il sera nécessaire d'explorer par d'autres travaux.
