This volume represents a first attempt at holistically classifying and mapping ecological regions across all three countries of the North American continent. A common analytical methodology is used to examine North American ecology at multiple scales, from large continental ecosystems to subdivisions of these that correlate more detailed physical and biological settings with human activities on two levels of successively smaller units. The volume begins with an overview of North America from an ecological perspective, concepts of ecological regionalization. This is followed by descriptions of the 15 broad ecological regions, including information on physical and biological setting and human activities. The final section presents case studies in applications of the ecological characterization methodology to environmental issues. The concept of Comprehensive Primary Health Care focuses on health system efforts to improve equity in health care access, community empowerment, participation of marginalized groups, and actions on the social determinants of health. Revitalizing Health for All examines thirteen cases of efforts to implement CPHC reforms from around the globe including Australia, Brazil, Democratic Republic of Congo, Iran, South Africa, and more. The findings presented in this volume originate from an international action-research set of studies that utilized triads of senior and junior researchers and knowledge users from each country’s public health system. Primary health care reform is an important policy discourse both at the national level in these countries and in the global conversations, and this volume reveals the similarities among CPHC projects in diverse national contexts. These similarities provide a rich evidence base from which future CPHC reform initiatives can draw, regardless of their country.

In Cooking Data Crystal Biruk offers an ethnographic account of research into the demographics of HIV and AIDS in Malawi to rethink the production of quantitative health data. While research practices are often understood within a clean/dirty binary, Biruk shows that data are never clean; rather, they are always “cooked” during their production and inevitably entangled with the lives of those who produce them. By examining public discourses and private narratives about infection, illness and death, this work reveals the contradictions between the lived experiences of AIDS suffering on the one hand, and biomedical certainties on the other.
