La recomposition consiste à mettre à disposition les instruments chirurgicaux pour les chirurgiens ou soignants selon un listing prédéfini d'instruments. Elle représente une étape entièrement humaine, difficile, chronophage et sensible. Le facteur humain étant une source inévitable d'erreurs, cette étape peut donc engendrer des non-conformités avec un risque potentiel pour le patient. L'objectif de ce travail est de réaliser un état des lieux des outils de recomposition et de mettre en place un nouvel outil pédagogique. Les outils informatiques sous la forme des logiciels de traçabilité permettent un accès facilité aux listings des compositions et intègrent des documents multimédia (photos, vidéos..). Ces éléments sont d'une aide précieuse pour les agents de stérilisation. De plus, grâce à la traçabilité à l'instrument, chaque dispositif médical est suivi de manière individuelle au sein des compositions. La formation à la recomposition des agents de stérilisation est primordiale. Des outils pédagogiques sont disponibles sous plusieurs formes (plateau de jeu, escape game, cartes...). En intégrant le jeu, ils favorisent l'intérêt des participants. Au sein de notre établissement, un outil destiné à l'apprentissage de l'instrumentation chirurgicale a été créé sous la forme d'un quiz. Au total, 65 questions portant sur 11 catégories d'instruments ont été définies. Cet outil est apprécié de l'équipe puisqu'il permet un moment de partage sans notion d'évaluation. Suite aux remarques issues du questionnaire de satisfaction, une nouvelle catégorie d'instruments destinée à la chirurgie ophtalmologique a été créée. Afin d'utiliser les nouvelles technologies, une adaptation sous format connecté a été affichée dans l'unité. L'ensemble des outils informatiques, technologiques et pédagogiques permettent d'obtenir une qualité de recomposition conforme aux attentes des chirurgiens/soignants. Cette étape encore humaine doit donc faire l'objet d'ingéniosité de la part des équipes pharmaceutiques pour créer de nouveaux outils toujours plus ludiques et toujours plus modernes.

Le Centre Hospitalier Universitaire (CHU) de Toulouse est engagé dans un projet de stérilisation centrale délocalisée, destiné à réunir les deux unités de stérilisations situées sur deux sites distincts. Cette nouvelle structure rentre dans le cadre d’une mise en conformité du processus de production des dispositifs médicaux stériles (DMS), dont notamment les opérations de lavage et de recomposition, actuellement effectuées par les blocs opératoires et services de soins. Ce transfert d’activité suscite des inquiétudes chez les chirurgiens du bloc d’ophtalmologie de l’hôpital Purpan par rapport à la prise en charge des instruments de microchirurgie. L’objectif de satisfaction de ce client nécessite donc une collaboration afin d’obtenir, d’une part, une bonne maîtrise des étapes précédant la stérilisation et, d’autre part une organisation logistique adaptée à l’activité opératoire. Ainsi, sur 370 références d’instruments d’ophtalmologie répertoriées, 13 références sont lavées à l’aide d’une compresse imbibée de détergent-désinfectant, 68 nécessitent une phase de lavage manuel, 41 sont immergées obligatoirement dans la soude. L’analyse du taux de rotation des instruments indique que 80% des intitulés d’instruments ou boîtes sont stérilisés moins de 1 fois par semaine (à 5 jours ouvrés) et que 45% des interventions se terminent avant 11 heures. Les résultats des études quantitatives et qualitatives effectuées au bloc opératoire et au service de stérilisation ont permis de dégager des axes de travail, en vue de la centralisation, et applicables à la prise en charge des autres blocs opératoires du CHU.

The Production and Distribution of Knowledge in the United States marked the beginning of the study of our postindustrial information society. Austrian-born economist Fritz Machlup had focused his research on the patent system, but he came to realize that patents were simply one part of a much bigger "knowledge economy." He then expanded the scope of his work to evaluate everything from stationery and typewriters to advertising to presidential addresses--anything that involved the activity of telling anyone anything. The Production and Distribution of Knowledge in the United States then revealed the new and startling shape of the U.S. economy. Machlup's cool appraisal of the data showed that the knowledge industry accounted for nearly 29 percent of the U.S. gross national product, and that 43 percent of the civilian labor force consisted of knowledge transmitters or full-time knowledge receivers. Indeed, the proportion of the labor force involved in the knowledge economy increased from 11 to 32 percent between 1900 and 1959--a monumental shift. Beyond documenting this revolution, Machlup founded the wholly new field of information economics. The transformation to a knowledge economy has resonated throughout the rest of the century, especially with the rise of the Internet. As two recent observers noted, "Information goods--from movies and music to software code and stock quotes--have supplanted industrial goods as the key drivers of world markets." Continued study of this change and its effects is testament to Fritz Machlup's pioneering work.

In Reign of Virtue, Miranda Pollard explores the effects of military defeat and Nazi occupation on French articulations of gender in wartime France. Drawing on governmental archives, historical texts, and propaganda, Pollard explores what most historians have ignored: the many ways in which Vichy's politicians used gendered images of work, family, and sexuality to restore and maintain political and social order. She argues that Vichy wanted to return France to an illustrious and largely mythical past of harmony, where citizens all knew their places and fulfilled their responsibilities, where order prevailed.
