En France, les traitements de substitution aux opiacés (TSO), instaurés en 1996, ont fait la preuve de leurs bénéfices, mais l'investissement des médecins généralistes (MG) dans ce domaine reste limité (97% des prescriptions émanent de moins de 50% des MG).Une revue de littérature analyse les obstacles freinant l'implication des MG français dans le suivi de cette addiction. Puis, une enquête transversale descriptive, au sein des MG angevins installés (n= 100/175 ; 57% de réponse), étudie la proportion de médecins investis, prêts à s'investir, ou ne souhaitant pas s'investir dans ce domaine. Un second questionnaire, envoyés aux participants ne souhaitant pas s'impliquer dans les TSO (n= 38/51 ; 74,5% de réponses), analyse - spécifiquement dans ce sous groupe - les facteurs qui font obstacles à cette implication. Dans leur grande majorité, les MG ne souhaitant pas s'investir dans les TSO, ont conscience de leur rôle important dans la prise en charge des addictions, et leur regard, sur les patients héroïnomanes, comporte de l'empathie. Mais, les appréhensions exprimées sur le suivi de ces patients relèvent d'une représentation traditionnelle du patient toxicomane (crainte de l'effet boule de neige, de la violence, du trafic de médicaments, du patient non motivé, manipulateur). L'analyse des réponses met en évidence des obstacles sous-jacents (non perçus par les MG comme étant importants) : la méconnaissance des objectifs de réduction des risques des TSO (la majorité des MG n'envisage que l'objectif du sevrage), un manque de formation sur l'accompagnement au changement, et la méconnaissance des structures locales spécialisées en toxicomanie.

Markets of Sorrow, Labors of Faith is an ethnographic account of long-term recovery in post-Katrina New Orleans. It is also a sobering exploration of the privatization of vital social services under market-driven governance. In the wake of Hurricane Katrina, public agencies subcontracted disaster relief to private companies that turned the humanitarian work of recovery into lucrative business. These enterprises profited from the very suffering that they failed to ameliorate, producing a second-order disaster that exacerbated inequalities based on race and class and leaving residents to rebuild almost entirely on their own. Filled with the often desperate voices of residents who returned to New Orleans, Markets of Sorrow, Labors of Faith describes the human toll of disaster capitalism and the affect economy it has produced. While for-profit companies delayed delivery of federal resources to returning residents, faith-based and nonprofit groups stepped in to rebuild, compelled by the moral pull of charity and the emotional rewards of volunteer labor. Adams traces the success of charity efforts, even while noting an irony of neoliberalism, which encourages the very same for-profit companies to exploit these charities as another market opportunity. In so doing, the companies profit not once but twice on disaster.

There are few areas of society today that remain outside the ambit of policy processes, and likewise policy making has progressively reached into the structure and fabric of everyday life. An instrument of modern government, policy and its processes provide an analytical window into systems of governance themselves, opening up ways to study power and the construction of regimes of truth. Life in Crisis tells the story of Médecins Sans Frontières (Doctors Without Borders or MSF) and its effort to "save lives" on a global scale. Begun in 1971 as a French alternative to the Red Cross, the MSF has grown into an international institution with a reputation for outspoken protest as well as technical efficiency. It has also expanded beyond emergency response, providing for a wider range of endeavors, including AIDS care. Yet its seemingly simple ethical goal proves deeply complex in practice. MSF continually faces the problem of defining its own limits. Its minimalist form of care recalls the promise of state welfare, but without political resolution or a sense of well-being beyond health and survival. Lacking utopian certainty, the group struggles when the moral clarity of crisis fades. Nevertheless, it continues to take action and innovate. Its organizational history illustrates both the logic and the tensions of casting humanitarian medicine into a leading role in international affairs.
