Acute pancreatitis with its different facets and complexity poses as a fascinating area of research. A distinct clinical entity for more than ten decades, acute pancreatitis continues to contribute significantly to the morbidity, mortality, and cost of treatment. While there is a need to advance the "frontiers of its clinical management" as is substantiated by considerable evidence, this thesis aims to integrate and better understand the concept of patient-centred medicine in an acute pancreatitis setting. Quality of life, particularly during hospitalisation, is the primary endpoint of all the three studies presented in this thesis. The first study is titled "Quality of Life after Acute Pancreatitis: a systematic literature review and meta-analysis"; the second study "The Effect of Enteral Nutrition versus Conventional Nil-By- Mouth on Quality of Life after Acute Pancreatits"; and the third study "The Effect of Oral Feeding Intolerance on Quality of Life of Patients with Acute Pancreatitis". Studies two and three are dedicated to addressing the gaps identified in the systematic literature review and studying the effect of nasogastric tube feeding (one of the optimal methods of administering enteral nutrition, the most promising supportive care modality) and nil-by-mouth (the conventional nutritional management of acute pancreatitis), and oral feeding intolerance (a common complication of nutrition management in acute pancreatitis patients) on quality of life, respectively. Although it was hypothesised that nasogastric tube feeding would result in a better quality of life than the conventional nil-by-mouth treatment, results obtained from the study did not support the same. Patients on nasogastric tube feeding, however, reported feeling more helpless than those on nil-bymouth. This was the only quality of life subscale found to differ significantly between the two groups. The third study addresses the effect of oral feeding intolerance on quality of life. Results obtained showed that quality of life was significantly impaired in patients who developed oral feeding intolerance compared with those who did not. Lastly, overall quality of life was observed to improve from time of hospital admission to discharge and from hospital discharge to both one and four weeks follow-up in both the groups.

The nutritional management of acute pancreatitis has evolved dramatically over the past few decades from the early regimen of 'pancreatic rest' to the modern paradigm of early enteral nutrition, or 'gut rousing'. This modern regimen has significantly improved clinical outcomes such as reducing infectious complications, multi-organ failure, and mortality for patients with acute pancreatitis; however the mechanisms behind these benefits remain largely unknown. In addition, despite these improvements, this has not eradicated the presence of a common nutrition-related complication; oral feeding intolerance (OFI). Characterised by pain relapse, nausea and/or vomiting, OFI has been associated with greater length of hospital stays and costs. As substantiated by literature there is a need for accurate, readily-available predictive markers that are capable of predicting OFI. Recently it was discovered that adipokines, mediators involved in inflammation and metabolism, have an early role in predicting the clinical severity of acute pancreatitis, thus it was hypothesised that adipokines may also provide insight into the mechanisms behind the benefits of early enteral nutrition, and may be predictors of other clinical outcomes. The aims of the study were to first investigate the effect of enteral nutrition on adipokine levels early in the course of acute pancreatitis, by comparing levels between patients receiving nasogastric tube feeding (a promising nutritional support therapy avenue) and those receiving nil-by-mouth (the conventional regime). The second aim was to investigate if adipokines could predict the primary outcome, OFI, and two secondary outcomes, systemic inflammatory response syndrome and prolonged hospitalisation, which often occur in acute pancreatitis. The five adipokines measured were adiponectin, leptin, omentin, resistin, and visfatin. Results obtained showed that two adipokines, omentin and leptin, were significantly higher in patients who received enteral nutrition compared to those who were nil-by-mouth. Literature shows that this may mediate the beneficial effects of early EN by having an anti-inflammatory effect, and may be protective against the development of insulin resistance and diabetes due to the role of these adipokines in insulin signalling pathways. Second, while none of the adipokines were significant predictors of OFI, resistin, a pro-inflammatory adipokine, was predictive of a prolonged hospitalisation; which could serve as a useful clinical indictor for patients who require more intensive monitoring. Future research should be directed towards understanding what role the increased levels play and whether they are responsible for the therapeutic benefits of enteral nutrition. Additionally, research should be directed towards determining the levels of omentin during AP and comparing them in differing levels of severity, to understand the role of this relatively new adipokine in AP. There is also great demand for research to investigate the mechanisms that cause and perpetuate OFI, to refine optimal refeeding practice and to ultimately understand how to prevent OFI.

Pancreatitis is a serious condition that occurs when the pancreas becomes inflamed. The pancreas is an organ that produces insulin and digestive enzymes. In the majority of patients, the illness settles over a few days but in 25% of cases it is more severe and associated with organ failure or pancreatic necrosis, requiring critical care and a prolonged hospital stay. The pancreas can become inflamed if the digestive juices, called enzymes, attack the gland itself, causing damage to the tissues of the pancreas. Pancreatitis can be an acute or a chronic condition. Common causes of pancreatitis include gallstones that form in the nearby gallbladder and travel to the pancreas via the common bile duct; long-term, heavy alcohol use; and certain conditions such as cystic fibrosis.Management of acute pancreatitis in the early stages is supportive. Acute pancreatitis is marked by the onset of severe pain in the upper abdomen that may spread to the back. With proper treatment, acute pancreatitis can be managed and resolved, although the condition and its complications may be life threatening in some cases. About 200,000 people in the United States are hospitalized each year due to acute pancreatitis.Chronic pancreatitis is a long-term condition that can result in permanent damage. In this condition the pancreas is slowly destroyed until it can no longer produce the important digestive enzymes. Treatment may include a prescription for synthetic pancreatic enzymes.Some complications are common to acute and chronic pancreatitis such as malnutrition caused by digestive problems, diabetes, which occurs in up to 80% of those with chronic pancreatitis, and accumulation of fluid within local collections (pseudocysts), in the abdomen (ascites) or chest (pleural effusion). Managing all these complications may be difficult because of ongoing comorbidities and social problems, such as alcohol or opiate dependence.People with pancreatitis are at long-term risk of nutritional problems and diabetes, and also have an increased risk of pancreatic cancer, which is even higher in people with hereditary pancreatitis. It is necessary to identify those who need to be followed up and what tests are required.Pancreatitis is a serious and complex condition. It causes immense suffering, can have a severe effect on quality of life and may result in reduced life expectancy. In the past, there has been lack of knowledge on how to manage pancreatitis and this has resulted in clinicians avoiding those with the disease and conflicting advice being offered.
