Le cancer du sein est la forme la plus répandue de cancer chez la femme. Grâce aux programmes de dépistage, le taux de patientes présentant un cancer du sein à un stade précoce s'accroît, il est actuellement de 50 %. La chirurgie reste alors le premier traitement, avec des taux de guérison variables (environ 40%). Il n'existe pas de manière fiable pour essayer d'identifier les patientes qui seront guéries et celles qui feront une rechute. C'est pourquoi, une thérapie adjuvante est habituellement préconisée après l'acte chirurgical afin de détruire toutes les cellules cancéreuses pouvant persister sur le site primitif. Le traitement de rechutes, qui surviennent chez 60% des patientes, fait appel essentiellement à la thérapie hormonale et à la chimiothérapie. L'hormonothérapie est l'un des traitements du cancer du sein. Il s'agit d'un traitement adjuvant, c'est à dire qu'il permet d'éviter les récidives, il complète ou non un traitement chirurgical ou une chimiothérapie. En effet, la glande mammaire est tout au long de la vie soumise à l'influence des hormones sexuelles (estrogènes et progestérone). Le cancer du sein peut se développer sous l'influence de ces hormones et y être plus ou moins sensible. Les antiaromatases constituent actuellement un réel progrès à tous les stades de la maladie par rapport au tamoxifène, qui était jusqu'à présent le " gold standard " que ce soit en termes d'efficacité ou de profil de tolérance. En effet, le tamoxifène administré en traitement adjuvant pendant 5 ans chez des patientes avec un cancer du sein ayant des récepteurs hormonaux positifs a été pendant plus de 20 ans le traitement standard international. Il entraîne une réduction de risque de décès de 25 %. Plus récemment, des résultats d'essais comparant un traitement adjuvant par inhibiteurs d'aromatase et par tamoxifène ont été rapportés. Ils montrent une efficacité des inhibiteurs d'aromatases supérieure à celle du tamoxifène sur le risque de rechute, qu'il s'agisse d'études comparatives (anastrozole) ou séquentielles (anastrozole, létrozole et exémestane). Mais plusieurs questions restent aujourd'hui sans réponse. -la durée optimale de traitement. -la définition de la séquence optimale de traitement et l'intérêt d'un traitement séquentiel (antiestrogènes suivi d'inhibiteurs de l'aromatase ou la séquence inverse). -et enfin la tolérance à long terme. Il est par ailleurs important que l'évaluation des anti-aromatases se poursuive pour disposer de plusieurs essais, avec des résultats à long terme, pour répondre à chacune des questions posées.
