81% des français désirent mourir à domicile mais seulement 25% y parviennent. La présence d’un aidant naturel est indispensable pour que ce projet aboutisse. L’objectif principal de cette étude était de définir les besoins des aidants naturels accompagnant un proche en fin de vie hospitalisé à domicile. MATERIEL ET METHODE : Une étude qualitative a été menée à partir de dix entretiens semi- dirigés d’aidants ayant accompagné un proche en fin de vie à domicile jusqu’au décès via le service d’hospitalisation à domicile Val-de-Loire. RESULTATS : L’aidant est rarement à l’origine de l’hospitalisation à domicile mais l’accepte pour respecter la volonté du malade et obtenir un soutien médical. La place du médecin généraliste est variable auprès du patient mais il est souvent un soutien de l’aidant. La disponibilité 24h/24 du personnel paramédical et médical est indispensable pour sécuriser la famille. Ces soignants forment l’aidant aux soins et le relaient parfois en cas de surmenage. Les rôles de l’aidant sont multiples : présence permanente, tâches domestiques, soins du malade, prises de décisions. Les conséquences pour lui sont psychologiques, liées aux angoisses face à la maladie et la mort ; physiques, dues à un manque de sommeil et la négligence de sa propre santé, et professionnelles. Des aidants déplorent un manque d’informations et de soutien. Des éléments positifs ressortent, principalement un accompagnement satisfaisant du malade jusqu’au décès. CONCLUSION : L’aidant a besoin d’un cadre rassurant et de bases préétablies sur lesquelles il pourra se reposer durant l’accompagnement. Cela passe par l’anticipation du retour à domicile et des difficultés futures potentielles ainsi qu’une information et un soutien psychologique réguliers.
