This volume looks at how northern European governments of the eighteenth and nineteenth centuries coped with the needs of the poor, whilst balancing any new measures against the perceived negative effects of relief upon the moral wellbeing of the poor and issues of social stability. Taken together, the essays in this volume chart the varying responses of states, social classes and political theorists towards the great social and economic issue of the age, industrialisation. Its demands and effects undermined the capacity of the old poor relief arrangements to look after those people that the fits and starts of the industrialisation cycle itself turned into paupers. The result was a response that replaced the traditional principle of 'outdoor' relief, with a generally repressive system of 'indoor' relief that lasted until the rise of organised labour forced a more benign approach to the problems of poverty.

Since George Rosen’s comprehensive History of Public Health, first published in 1956, there has been no internationally comparative survey of the subject. Mutualism and health care, newly available in paperback, presents the first comprehensive account of a major innovation in hospital funding before the NHS. The voluntary hospitals, which provided the bulk of Britain’s acute hospital services, diversified their financial base by establishing hospital contributory schemes. Through these, working people subscribed small, regular amounts to their local hospitals, in return for which they were eligible for free hospital care. The book evaluates the extent to which the schemes were successful in achieving comprehensive coverage of the population, funding hospital services, and broadening opportunities for participation in the governance of health care and for the expression of consumer views. It then explores why the option of funding the post-war NHS through mass contribution was rejected, and traces the transformation of the surviving schemes into health cash plans. This is a substantial investigation into the attractions and limitations of mutualism in health care. It is highly relevant to debates about organisational innovations in the delivery of welfare services.
