Excerpt from Medical Science, Vol. 3: Abstracts Reviews, October 1920 Aetiology. In an article on the specificity of acute articular rheumatism, Grenet reports having observed outbreaks of five distinct epidemics among soldiers within a limited area. The disease occurred within a zone of trenches occupied by certain regiments and also appeared in their rest camps, where they came in contact with second line troops who became infected in their turn. The immunity of a regiment after occupying the former cantonment of another regiment in which cases had occurred tended to show that contamination was not caused by the soil or by the quarters, but by direct contact. Further evidence of the contagious character of acute rheumatism is afforded by Grenet in the history of the following case. A man who had hitherto escaped rheumatism, but belonged to a regiment in which an outbreak had occurred, was suddenly separated from it and admitted to a hospital for a wound. Twenty-six days after admission he had a definite attack of acute rheumatism, and six days later two wounded soldiers occupying adjacent beds had similar attacks. While admitting that the contagiousness of acute rheumatism is of slight degree, so that it usually escapes notice, Grenet points out that it is an argument of the highest value in favour of the specificity of the disease. Frauenthal declares that teeth and gums as sources of infection in rheumatism are overestimated, and that much harm is done by the promiscuous extraction of teeth owing to the incompetent interpretation of X-rays. Any acute or chronic infection of the gastro-intestinal canal, genito-urinary tract, or respiratory system may give rise to an acute or chronic inflammation of the joints. According to Lambert, the percentage of acute rheumatism among the total admissions to the Bellevue Hospital, New York, has steadily diminished from 1907, when it was 2.45, to 1919, when it was 0.521. He regards dental hygiene more than any other one factor as responsible for this diminution, and thinks it probable that tonsillectomy is also in part a factor. Symptomatology. In a series of 73 cases of rheumatic fever in soldiers, studied by White at U.S. Base Hospital No. 6, 51 per cent, gave a previous history of rheumatic fever, and 40 per cent, showed large or ragged tonsils. The joints involved in order of frequency were knee, ankle, shoulder, wrist, foot, elbow, hand, and hip. Acute pericarditis, which was usually very transient, was found in 10 per cent., and acute pleurisy in 19 per cent. There was evidence of mitral endocarditis in 40 per cent., and of combined mitral and aortic endocarditis in 8 per cent. Most of these cases were apparently of long standing.
